We can never know when a family member will get sick or be injured. IlIness and injury happen unexpectedly. But the hospital is no longer the only place for treating patients. Some hospital techniques and routines can now be adapted at home that even some seriously ill patients can be taken care of within the comfortable surroundings of the patient's own home. The home is actually an excellent place for treatment and recovery when the patient's medical and nursing needs can be adequately met there.
Home nursing focuses on the patient. It is concerned with the patient's needs and well-being. In the home, the patient is cared for as a loved one rather than a sick individual because of the fact that home nursing encourages open communication among family members. The nursing career structure involves a broad spectrum of work and responsibilities in different health settings. The home nurse can upgrade his/her qualifications to become either a health-care professional. i.. the licensed medical personnel, or become a health-care worker who gives informal or nonmedical care Health-care professionals involved in the formal care to patients are primarily the registered or licensed practical nurses. Has Oregon Found a Better Categorical Rule?
Although a right to assisted suicide for the terminally ill can be seen as a better categorical rule than permitting assisted Suicide for no one, there is a serious question as to whether this is actually the case. A number of commentators argue that it is unworkable to base a legal (or ethical) rule on the presence or absence of terminal illness, that the distinction between terminal and nonterminal illness does not give us a bright-line distinction in the way that the distinction between treatment withdrawal and assisted suicide does.
Some ot the arguments against a rule based on terminal illness lack sufficient merit. For example, it has been argued that there is no principled basis for permitting assisted suicide for terminal people with very serious, nonterminal illnesses. Indeed, if an important goal of a right to die is to permit people to avoid intolerable suffering, then people who have a longer time in which to live arguably have a stronger basis for having a right to die.
If our right-to-die rules were based strictly on case-by-case judgments, this objection would be very persuasive. However, as I have discussed, the distinction between treatment withdrawal and assisted suicide is dictated not simply by the usual theoretical arguments but also by the moral con-lcerns involved when the state makes life-and-death decisions based on its own assessment of people's quality of life. This moral concern takes us to categorical rules for right-to-die law. By their nature, categorical rules are impertect reflections of important moral principles. Accordingly, it is not surprising that we can find people in one category who we think should fall into another category. That is precisely the compromise that a society makes when it adopts categorical rules.
The more important concern about a line based on terminal illness is the point that there is some uncertainty about Iife expectancy predictions, even for seriously ill persons, Although we can precisely define terminal illness in terms of a life expectancy of six months or less, we cannot precisely determine when a person meets that definition. We have all probably known people who lived for many months after they were supposed to have died. Among patients certified for hospice coverage under Medicare as having a life expectancy of less than six months, 15 percent survived for more than six months. Accordingly, some patients may choose assisted suicide on the mistaken impression that they have only a few months to live. This is a serious concern, and it may prove to be a real obstacle to a widespread acceptance of assisted suicide, even if the right is limited to terminally ill persons.
On the other hand, there are good reasons to believe that it is not an insurmountable problem. First, concerns about uncertainty can be answered to a large extent by limiting assisted suicide to cases in which predictions of survival can be made with a high degree of certainty. For example, predictions about life expectancy are more reliable for patients with metastatic cancer than for patients with congestive heart failure, emphysema, or end-stage liver disease.Second, when making predictions about life expectancy in dying patients, physicians are much more likely to overestimate than to underestimate how much longer the patient will live. Third, our experience with treatment withdrawal suggests that uncertain prognoses are not a truly serious concern. Imprecise predictions of lite expectancy are also a concern for withdrawals of treatment. Patients may refuse life-sustaining treatment because they have mistakenly been told that they are terminally ill or that they will be dependent on artificial life support for the rest of their lives. For example, my example of a "permanent-ventilator" patient, Mr. Poe, might not really have been irreversibly dependent on a ventilator. Since there does not seem to be a real problem with patients ending their lives prematurely by treatment withdrawal because of mistaken prognoses, it is unlikely that there will be a real problem with patients ending their lives prematurely by assisted suicide.
Importantly, the point of having a categorical rule to distinguish between morally justified and morally unjustified deaths is not simply to have a rule that can be easily and precisely applied. Rather, the primary concern is to avoid having representatives of the state decide on a case-by-case basis whether someone's life is so miserable that it need not be prolonged. Even if it is difficult to determine whether someone is ternminally ill, the determination does not require case-by-case judgments about the value of someone's life. It only requires case-by-case judgments about the expected length of the patient's life.
In short, although there is some reason to question whether terminal illness provides a sufficiently bright line for purposes of a right to assisted suicide, there is very good reason to think that it does. On this question, Oregon's experience with a right to assisted suicide for the terminally will be instructive.
The Role of Rules in Medical Ethics and Law
Despite the amount of attention to the choice between case-by-case judgements and rule-based decision making by some scholars, the importance of that choice has often been neglected in discussions of specific issues in medical ethics. In particular, discussants commonly do not give full recognition to the moral concerns that arise when principle is translated into rules or judgments. Rules or
judgments are frequently seen as fairly straightforward applications of an important moral principle. For example, if one considers it immoral for physicians to cause their patients' deaths, one might look favorably on a rule that prohibits physician-assisted suicide for all patients or a judgment that denies assisted suicide to a particular patient. In such a case, the rule or judgment codifies one way in which the underlying principle applies to physicians' practices. The principle against causing patient deaths might lead to other straightforward rules, including a prohibition of euthanasia and a prohibition of the negligent practice of open-heart surgery.
Although it is important that rules concretize key under lyıng principles,they often serve the other important goals that I have discussed. Drivers are in a much better position to predict whether they will be pulled over for speeding if the operative rule is "The speed limit is sixty-five miles per hour than if the operative rule is "Drive safely." At the same time, it is much easier for a police officer to decide when someone is speeding when the operative rule is based on a speed limit of sixty-five miles per hour than when it is based on the requirement that drivers proceed safely.
When the other roles of rules are neglected, rules may seem misguided. That is, ifa rule is thought to represent a straightforward application of principle, it will often be misunderstood, as with the previous example of a law permitting abortion for any reason before the fetus is viable. In the remainder of this part of the book, I will consider the importance of generally valid rules for end-of-life decision making. In doing so, I will argue that important moral principles in end-of-life decision making are captured by simple, categorical rules, much as the social concern about safe driving is captured in a simple, categorical rule of a speed limit, and the social concern about voter maturity is captured in a simple, categorical rule of a minimum voting age. Specifically, I will argue that the distinction between physician-assisted suicide and the withholding or withdrawing of the life-sustaining treatment exists because it provides a simple,"bright-line" way to distinguish between morally justified and morally unjustified patient deaths. That is, permitting treatment withdrawal (or withholding) and prohibiting suicide assistance is essentially a "proxy for permitting morally justified patient deaths and prohibiting morally unjustified patient deaths, just as permitting speeds below sixty-six miles and hour and prohibiting speeds above sixty-five miles per hour is a proxy for permitting safe driving and prohibiting unsafe driving. This proxy rule for end-of-life decision making exists because it would not be feasible to make case-by-case judgments regarding the moral acceptability of a patient's death.
By understanding the distinction between treatment withdrawal an suicide assistance in this way, we can understand both why scholars a correct to criticize the distinction between treatment withdrawal and Suicide assistance from the perspective of underlying principle and why still has made sense for societies to adopt the distinction. In other words, although the distinction between treatment withdrawal and suicide assistance may seem problematic in terms of underlying moral principle, it is in fact less problematic when one takes into account the moral concerns involved in translating principle into practice.
By understanding the assisted suicide-treatment withdrawal distinction in terms of its proxy role, we can also understand how a limited right to physician-assisted suicide can be recognized without any change in society's theory about what constitutes a morally justified patient death. Relaxing the prohibition against assisted suicide can be explained in terms of finding a better proxy for underlying principle while still accounting for the moral concerns involved in translating principle to practice.
In short, I have chosen to discuss the distinction between physician assisted suicide and treatment withdrawal because it demonstrates the importance of generally valid rules in medical ethics and law, and it does so in the context of a critical life-and-death issue.
The Importance of Generally Valid Rules in Implementing Moral Principle
WHEN MORAL principle is translated into practice, a critical question is whether to rely primarily on case-by-case judgments or primarily on rulebased decision making. For example, if organs for transplantation are to be allocated so that benefit to patients is maximized,' physicians could make a case-by-case judgment for each organ to decide which patient would gain the most benefit from a transplant and therefore would be the best candidate for the organ. Alternatively, physicians could rely on benefits-based rules to determine the recipient of each organ, with the idea that the rules would usually identify leading candidates for transplantation but not necessarily the best candidate each time.
I will say more about the choice between case-by-case judgments and broadly applicable rules, but an important feature of rules is their general validity. That is, rules will typically, but not always, serve their underlying principles. The general validity of a rule can be illustrated as follows. Assume that the goal in transplantation is to give organs to those patients who will live the longest with their new organs. Transplant surgeons might therefore exclude from consideration for kidney transplantation patients who are heavy drinkers of alcohol.' Such a rule would generally ensure longer survival of kidney transplant recipients, but it would also deny transplants to patients with heavy alcohol use who do unexpectedly well and ive longer than many other patients who would receive a kidney transplant.
When generally valid rules are used, they can appear to be misguided. Since they are by definition imperfect proxies for their underlying principles, they will at times yield results that seem unfair. The kidney failure patient with heavy alcohol use whose life is not shortened by the drinking will not have an opportunity for a transplant. Similarly, if state laW requires individuals to reach the age of adulthood before exercising decision making capacity, adolescents of advanced maturity may be denied decision-making authority that they deserve." Because of their imperfection, generally valid rules will often be criticized as interior to case-by-case judgments.
Such criticism, however, misses an important reason for using generally valid rules rather than individualized judgments. Often, the moral concerns that arise in the translation of principle to practice make case-by-case judgments undesirable. Thus, for example, aspects of abortion law can be misunderstood if the move from principle to practice 1s not considered. When the law permits women to have an abortion for any reason if the fetus is not viable, people frequently object to the law on the ground that it indicates that abortions are always morally permissible when the fetus is not viable. As these opponents observe, the morality of an abortion may depend not only on the fetus's viability but also on the woman's reasons for wanting an abortion. If the woman chooses an abortion to protect her health, we have a very different case than when a woman chooses an abortion because she is angry with the father of the fetus and she wants to get back at him.' However, the law permits abortions for any reason before viability, not because reasons are irrelevant, but because it would not be feasible to distinguish between acceptable and unacceptable bases for an abortion. Women could easily lie about their reasons, and it would be too intrusive tor physicians to investigate the validity of the woman's explanation. In other words, it would not be possible to implement in practice the principle that a woman must have a morally valid reason for an abortion.
If one believes that abortions are permitted for any reason because one mistakenly thinks the law deems reasons morally irrelevant, one will misunderstand why the law has not qualified the right to abortion before viability in terms of the woman's reasons for choosing abortion." Proper understanding of abortion law requires consideration of both underlying principle and the translation of principle into practice.
In the remainder of this chapter, I will discuss in more depth why generally valid rules are often preferable to case-by-case judgments. I will also indicate how the distinction between physician-assisted suicide and withdrawal of life-sustaining treatment can be better understood in terms of using generally valid rules to accomplish the translation of principle into practice.

Today i will show you my flower photography .
I took 3 photos of different flowers .
I don't know what are their names but the last photo that i was taken is a flower of the (OKRA).
The photo below is a flower of vegetable (OKRA) which is my favorite vegetable ,

I just wanted to share my artwork ,
Maybe its not really nice but i tried again To join at the second time to **@Jane** artwork contest .😅 https://read.cash/@Jane
I only use 3 materials which is what i only have .
After I'm done to draw this, i decided to use an application to make this simple draw into more colorful in different effects.
Then i made it as GIF ,
The reason why i made or draw this one is because i had a lot of questions and thoughs in my mind that hasn't yet answered though i ask myself many times.
i know that it's really hard for me to get an answer without asking anyone or someone.
But i choose to always keep it in myself .
The feeling that when you're so very feeling down and you know it in yourself that you have to keep it only in yourself.
Feeling that even when you have difficulty thinking about things you can't get from someone else or ask anyone you would like to ask.
don't know what Is the exactly right word to explain It.
I just know I'm struggling.

so here i am ,
I have two reason why i draw this .
1) The reason why i chose to draw krillin from dragon ball Z because dragon ball is my favorite cartoon in television and i always loved to watch it.
2) i loved krillin from dragon ball Z because he was a very thoughtful and a very good friend to Goku until goku have a son name gohan . It was amazing because Krillin was not growing up and nothing was changed in his appearance.
