¿QUÉ ES EL HIPOTIROIDISMO?

Es una afectación en la cual la glándula tiroides no produce suficientes hormonas tiroideas, porlo que los niveles de las mismas se hallan disminuidas en el torrente sanguíneo.

Es una patología que no tiene curación, solamente control. Es más frecuente en mujeres entre los 20 y 30 años de edad.

TIPOS DE HIPOTIROIDISMO

- Hipotiroidismo primario: se debe a afectación propia de la glándula tiroides, éste puede ser congénito (déficit del desarrollo de la glándula) o adquirido (idiopático -es el más frecuente, causado por enfermedad autoinmune, llamado también "tiroiditis de Hashimoto"- o por yatrogenia -causada por extracción de la glándula o parte de ella, radioterapia en cabeza y cuello, etc.-).

- Hipotiroidismo secundario: Se debe a afectación de la hipófisis, lo que produce una disminución en la producción de TSH (hormoda estimulante de tiroides), en este caso la tiroides funciona normalmente, pero, el estímulo para la misma es ineficaz.

- Hipotiroidismo terciario: Se debe a afectación del hipotálamo, lo que provoca una disminución en la secreción de TRH (factor liberador de tirotropina), un estímulo para que la hipofisis libere TSH y ésta a su vez estimule a la glándula tiroides.

MANIFESTACIONES CLÍNICAS Y DIAGNÓSTICO

Los pacientes suelen cursar con las siguientes manifestaciones:

- Astenia (falta de ánimos).

- Bradilalia (habla lenta).

- Bradipsiquia (pensamiento lento).

- Bradicardia (frecuencia cardiaca lenta)

- Uñas quebradizas.

- Estreñimiento.

- Somnolencia.

- Pérdida del cabello.

- Aumento de peso.

- Cara redonda o en forma de luna llena.

- Alteraciones del sangrado menstrual.

- Impotencia sexual en hombres.

El diagnóstico se realiza mediante la exploración física y a través de parámetros del laboratorio (perfil tiroideo), donde se puede evidenciar un aumento de los niveles de TSH y disminución de las T3 y T4.

TRATAMIENTO

La levotiroxina es el tratamiento de elección para cualquier caso de hipotiroidismo, la dosis se debe ajustar cada 2 a 4 semanas, hasta lograr la dosis ideal para el paciente, la idea del tratamiento es tratar de mantener los valores de TSH normales.

WHAT IS HYPOTHYROIDISM?

It is a condition in which the thyroid gland does not produce enough thyroid hormones, so the levels of these hormones are decreased in the bloodstream.

It is a pathology that cannot be cured, only controlled. It is more frequent in women between 20 and 30 years of age.

TYPES OF HYPOTHYROIDISM

- Primary hypothyroidism: it is due to the thyroid gland's own affectation, it can be congenital (deficit in the development of the gland) or acquired (idiopathic -it is the most frequent, caused by autoimmune disease, also called "Hashimoto's thyroiditis"- or by iatrogenic -caused by extraction of the gland or part of it, radiotherapy in the head and neck, etc.-).

- Secondary hypothyroidism: It is due to involvement of the hypophysis, which produces a decrease in the production of TSH (thyroid stimulating hormone), in this case the thyroid functions normally, but the stimulus for it is ineffective.

- Tertiary hypothyroidism: It is due to involvement of the hypothalamus, which causes a decrease in the secretion of TRH (thyrotropin-releasing factor), a stimulus for the pituitary to release TSH and this in turn stimulates the thyroid gland.

CLINICAL MANIFESTATIONS AND DIAGNOSIS

Patients usually present with the following manifestations:

- Asthenia (lack of mood).

- Bradylalia (slow speech).

- Bradypsychia (slow thinking).

- Bradycardia (slow heart rate).

- Brittle nails.

- constipation

- drowsiness

- hair loss

- weight gain

- Round or full-moon shaped face.

- menstrual bleeding disturbances.

- Sexual impotence in men.

Diagnosis is made by physical examination and through laboratory parameters (thyroid profile), where an increase in TSH levels and decrease in T3 and T4 can be evidenced.

TREATMENT

Levothyroxine is the treatment of choice for any case of hypothyroidism, the dose should be adjusted every 2 to 4 weeks, until achieving the ideal dose for the patient, the idea of treatment is to try to maintain normal TSH values.
