Osteomalacia, Diagnosis and it's treatment *****Osteomalacia***** In adults, vitamin D deficiency results in Osteomalacia which is characterised by a wide scam of unmineralised matrix (osteoid) living the bone surfaces. Vitamin resistant rickets is an inherited disorder which is apparently caused by defective receptors for 1,25(OH)2 D3 The common presenting features are pain and mascular weakness.Pain ranges from a dull ache to severe pain.Sites frequently affected are ribs,sacrum,lower lumber vertebrae,pelvis and legs.Bone tenderness on pressure is common. Mascular weakness is often present and the patient may find difficulty in climbing stairs or getting out of a chair.A waddling gait is not unusual. There is rarefaction of bone and commonly translucent bands(pseudo fractures, ***Looser's zones***) often symmetrical at points submitted to compression stress. Common sites are the ribs,the axillary borders of the scapula, the public rami and the medical cortex of the upper femur.Looser's zones are diagnostic of Osteomalacia. *****Diagnosis***** : Radiograph of the wrist may show characteristic changes at the epiphyses.The outline of the joint is blurred and hazy and the epiphyseal line becomes broadened. Later in older children, as a result of decalcification of the metaphysis and the effects of movements and stresses the classical concave 'saucer' deformity is clearly shown radiographically.The diagnosis is supported by a raised plasma alkaline phosphatase and confirmed if plasma 25(OH)D is low. *****Treatment***** : A daily oral dose of 25-125 μg (1000-500IU) of vitamin D cures osteomalacia. Because of the risk of toxicity this should be reduced to 10 μg,the prophylactic dose,when plasma alkaline phosphatase has returned to normal and radiographs show that healing is established. Children can be given 1 ml of halibut oil. Advice on diet and general hygiene is needed.An adequate intake of calcium is essential. The best source is milk and at least 500 ml should be taken daily. An egg daily and butter or fortified margarine to increase the dietary intake of vitamin D are recommended.Children should enjoy playing in the sun. Marasmus and Marasmic Kwashiorkor *****Marasmus:***** Child reacts to the stress of PEM and secretes cortisol, which mobilises protein from muscle and subcutaneous tissue to amino acid pool resulting in wasting with no oedema and no hepatomegaly.Raised cortisol level lowers growth hormone and so the child is stunted.Marasmus is said to be well adapted to the stress of deficit in protein and calories.The signs and symptoms observed in marasmic child are the following : Severe growth retardation Loss of subcutaneous fat Severe muscle wasting The child looks appallingly thin and limbs appear as skin and bones Shrivelled body Wrinkled skin Bony prominence Associated vitamin deficiencies Failure to thrive Irritability, fretfulness and apathy Frequent watery diarrhoea and acid stools Mostly hungry but some are anorectic Dehydration Temperature is subnormal Muscles are weak Oedema and fatty infiltration are absent *****Marasmic Kwashiorkor:***** The child shows a mixture of some of the features of marasmus and kwashiorkor. Requirements and sources of IRON *****Requirements of IRON:***** The requirements are determined by iron-balance studies as in the case of other nutrients.This method, however, yield abnormally high values due to cumulative errors.Iron requirement is established by determining body iron loss through long-tern turnover studies in adult men using radio isotope of iron.Daily loss of body iron in well-nourished adult men has been estimated to be 12-15 μg/kg with an average of 14 μg/kg. This figure is also used for determining the basal loss of body iron in women and children.The total body iron requirement of different  groups is computed factorially as follows. *****Adult man***** : Basal loss of iron (through GI tract,urine and sweat)= 14 μg/kg *****Adult woman***** : Basal loss (14) + average loss of iron through menstrual loss of blood (16) = 30 μg/kg. *****Children***** : Basal loss (14) + growth + expansion of blood volume (15) + improved iron stores = 29 μg/kg. *****Pregnant woman :***** Basal loss (14) + foetal growth + expansion of blood volume (46) = 60 μg/kg. *****Lactating woman***** : Basal loss (14) + loss through breast milk (16) = 30 μg/kg. *****Sources of Iron:***** Rich sources of iron are cereals, millets,pulses and green leafy vegetables. Of the cereal grains and millets, bajra and ragi are very good source of iron.Iron from animal foods is better absorbed than from planet foods.Inclusion in our daily diet about 50 g of green leafy vegetables which are rich in iron can meet a fair proportion of iron needs. Iron content of food can be increased by cooking in iron vessels. Studies have shown that iron pots released significantly more available iron into food than the other pots.Food in villages is likely to be contaminated with iron from water,soil residues or dust.Cooking in iron pots is less expensive compared to iron fortification and this should be one of the strategies to prevent anaemia. ❤️..............End.............❤️ Thanks for reading. Possible Health Benefits of β-Carotene Today I want to discuss about some possible health benefits of β-Carotene.I hope everybody can learn many important things from here.So let's start: *****Cancer:***** Many epidemiological studies have found that a high intake of foods rich in β-Carotene is correlated with a reduced risk of certain cancers.The most definite correlation is with reduced risk of lung cancer.β-Carotene has also been associated with reduced risk of cancer of the female reproductive system, gastrointestinal system and mouth,gums and trachea.These are all cancers of epithelial cells.The beneficial effects attributed to β-Carotene may in some cases be due to other carotenoids or may be shared by all carotenoids. In man the administration of β-Carotene has been reported to reduce the number of micro nucleated buccal mucosal cells in tobacco chewers as also lower the frequency of chromosomal breakages in betel nut eaters. The mechanism by which β-Carotene acts as an anti carcinogen is not established. It has been suggested that this effect may be mediated through its well known property of being an effective quencher of singlet oxygen or that it may deactivate certain reactive molecules, by altering hepatic levels of some drug metabolising enzymes, thus preventing cell damage.It has been suggested that β-Carotene may act by enhancing the immune defense mechanism. *****Cardiovascular disease :***** β-Carotene may also be an important nutritional weapon in the fight against cardiovascular disease. Supplemental intake of β-Carotene has been shown to reduce the incidence of angina in a small sample of men who had previously had heart attacks. *****Cataracts:***** Cataracts are areas of opacity in the lens of the eye. Their formation has been linked to damage by oxidising agents and free radicals.People with low serum carotenoid levels had more than 5 and 1/2 times the risk of developing cataracts than those with high serum carotenoid levels.Although the lens of the eye contains little β-Carotene, it has been suggested that β-Carotene may protect against cataracts simply by decreasing the level of oxidative damage in the body as a whole. *****Gastric inflammation :***** Carotenoids have inhibitory effect on gastric inflammation induced by helico bacter pyroli by inhibiting the activation of transcription factor. Hypocalcaemia & Hypercalcaemia *****Hypocalcaemia***** ***Hypocalcaemia*** in clinical conditions is rarely caused by inadequate calcium ingestion. This may occur after operations on the thyroid gland,if too much parathyroid tissue is removed, or due to impaired alimentary absorption. It may also occur in patients with the malabsorption syndrome. When plasma ionic calcium is reduced, nerve and muscle become more readily excitable.Sensation of tingling and numbness may be present and on the motor side twitching of muscles,known as tetany,which may be followed by spasm,involuntary muscle contraction.The face,hands and feet are mainly affected ; characteristically the wrist and metacarpo phalangeal joints are flexed and the interphalangeal joints extended.The larynx may be affected and this causes a coarse stridor,harsh sound.Paresthesia of lips,tongue,fingers and feet may occur in ***hypocalcaemia***. *****Hypercalcaemia***** This occurs in adults  as a result of hyperparathyroidism or excessive doses of Vitamin D.***Hypercalcaemia*** can occur in infants as a result of high intakes of Vitamin D.It is best corrected by reducing the Vitamin D content of the diet,rather than by reducing the dietary calcium intake.It has also been reported in patients with peptic ulcer and impaired renal function treated with a milk diet and large doses of alkali.It may also occur in patients with cancer of the lung and at other sites due to "inappropriate" secretion of a parathyroid hormone - like substance by tumour cells. In ***hypercalcaemia***, gastrointestinal symptoms include anorexia,nausea,vomiting,constipation, abdominal pain.Renal stones and muscle weakness can also occur.***Hypercalcaemia*** can be reduced by treatment with calcitonin. Assesment of the neonate physical conditions After the baby is born, the umbilical cord is cut and separated from the mother. The period from birth to 2 weeks is called neonatal period. The following tests are performed if the baby continues to breathe normally after birth. *****Birth weight of the baby*******:** The weight of a healthy baby is usually 2.5-3.5 kg. If it is less than 2.5 kg, it is called low birth weight baby. Their risk of death is higher. *****Baby height*****: Generally baby height is about 50 cm. *****Measurement of the child's well-being: (APGAR Score):***** The Apgar test is checked twice, after 1 minute of birth and 5 minutes after birth. If the Apgar score is 8-10, it should be understood that the child is completely healthy. 80% of newborns have a score of 8. If the score is 4-6, the child should be kept specially. If the score is 3, it should be understood that the condition of the newborn is serious. A lower Apgar score increases the chances of infant death. *****Birth defects of the baby:***** After the birth of the baby, it is confirmed if there are any birth defects or not.For example: no esophagus or rectum, palate cut and lip cut etc. *****Postpartum Injury:***** During delivery, it is checked to see if there is any injury or swelling in the head, any injury to the limbs. *****Excretion*****: A healthy newborn will urinate within 24 hours and excrete within 48 hours. Some special moments of my childhood I was admitted to a kindergarten school when I was 5 years old. There we boys and girls studied together. Our school had English version. There were 10/15 students in each class. We had two teachers in each class. They wrote down in our diaries every day. And our parents used to teach us well at home. That is why almost everyone in the class got 96/97/98 marks out of 100 in all subjects.😋 When I was in class 1, all the girls used to sit with the girls but I used to sit between two boys. So one day I was doing a lot of mischief with my friends. Suddenly one of my friends hit me in the eye with a pen.And I kept crying. Then the teacher sat down me with the girls and punished the boy. The boy was in a lot of pain. Even the next day he was not at school anymore. I still remember this incident very well.I wish I could meet with him again😇 Another day our teacher was teaching us. When she finished writing on the board, she asked us to write down in the notebook. Meanwhile, there were no chairs where there were chairs every day. When Madam went to sit thinking that there was a chair, she fell down.After this incident, Madam started laughing. And everyone in the class was laughed.😅😅😂 I remember another incident of my school life very much. We had a class party in our school in every year. On that day we used to take flowers for the teachers. And we would go after a beautiful dress. At a class party like this, my friends and I were rejoicing after wearing new clothes. After we were dancing and singing all the time, it was our turn to take gifts.Then teacher gave us all beautiful mug. But unfortunately the mug fell from my hand and broke. And I started to cry. Then my mother told me "Don't cry, I will buy you another mug" then my crying stopped. I really miss those days very much.I wish I could go that time again.😍Thanks for reading friends. Chicken Chinese Vegetable Hey friends, How are you all? Hopefully everybody is well.Today I will show you how to cook Chicken Chinese Vegetable. It is a very tasty and healthy food for us. It has a lot of nutrients due to the presence of vegetables.This food is also very beneficial for children. So let's start..😇😇 *****Ingredients*****: 🍜 1/4 part of raw papaya 1 green capsicum 1 carrot 3 onions Soya sauce Black pepper 1 tea spoon of crushed garlic Ginger powder 1 tea spoon 3 green chillies 1/4 tea spoon of testing salt Chicken Water Cornflour *****Cooking Method :***** First you have to pour oil in the pan. Then you have to give half crushed garlic and ginger powder . When they become light brown, you have to mix the chicken. Then you have to stir the raw chilli with the chicken. When the chicken is cooked, you have to mic papaya and carrot in it. Then you have to mix one and a half cup of water. You can boil the vegetables before if you want. Then when it is cooked, you have to pour two tablespoons of soya sauce, onion and capsicum. Chinese vegetables cannot be softened at all. Something has to be kept hard. That means it will look hard but it will be cooked inside. Then mix two tablespoons of cornflour with half cup water and mix it with vegetables. Chinese vegetables are liquid to look at. Absolutely not frozen. So you can not give a large amount of cornflours.Then shake off the stove for a while. Your favourite chicken Chinese vegetable is now ready to eat. Today we will know about What is kidney and its location Kidney function Symptoms of kidney failure Kidney disease is one of the deadliest diseases in the world. So we need to know what is kidney, what is kidney disease , why kidney disease occurs, what to do about prevention and cure. The kidney is one of the most essential organs in the human body. The human body has two kidneys on either side just above the waist. In adulthood, a kidney is 11-13 cm long, 5-6 cm wide and 3 cm thick. A kidney weighs about 150 grams.However, the left kidney is slightly larger and slightly higher than the right kidney. Each kidney is made up of about 10 lakh nephrons. The nephron is the functional and structural unit of the kidney. When these nephrons are damaged for any reason, the kidneys quickly become useless. Kidney disease usually affects two kidneys at the same time. Numerous biochemical reactions are constantly taking place in our body. The contaminants produced by these reactions are absorbed into the bloodstream. And the kidneys filter the blood through its filters and expel contaminants (urea, uric acid, ammonia, creatinine, etc.) from the body with the urine.In this way the kidneys protect our body from toxic and harmful wastes. In addition, the kidneys have other functions. *****Kidney function*****: - The activities involved in the kidneys are- 1. Excretion of contaminants from the body through urine; 2. Maintain water balance in body and blood; 3. Helping to keep fixed the amount of blood components ; 4. To make vitamin ‘D’ effective; 5. Making renin; 6. Many people have kidney damage and new kidney transplantation is painful and expensive. Kidney transplantation and treatment are all painful. So the easiest way to avoid any disease is to know and be aware of the disease. Doctors say that prevention is better than getting sick. Let's know why kidney disease occurs. Her heart is in the bag Can anyone tell by looking at the picture of the lady what is in her backpack? No one can. You can't, even if I say so, you don't want to believe. Yes, I'm telling true . Listen then.She has her 'heart' in her bag. The most important organ of the human body is the heart which keeps our life alive at every moment. She carried that integral part around her neck. It's not a miracle, it's a real fact.This is one of the achievements of medical science. The name of the gentle woman is 'Selwa Hossain'. She is now fully healthy and can return to normal. She can laugh, she can cry. All her feelings and actions are normal. Six months ago from today, Selwa felt chest pain. Then she went to the regional hospital. There, realizing her critical condition, she was sent to a relatively good hospital. Doctors , considering her condition, sent her to the famous ‘Herfield’ hospital in Britain. By examining her there they make sure she can't keep her heart moving. This historic decision was taken after a board of doctors sat down and discussed with her family. The heart of her body is removed and a plastic chamber is placed there, which will act as the heart of the human body. And a ‘pump’ is needed to create air pressure by keeping this plastic chamber active. One motor is required to keep this pump running and two batteries are required to keep that motor running.And these batteries, pumps and machines she carried in her backpack. The pump in that bag opens a plastic chamber placed in her chest with air through two white pipes, which keeps the blood flow in her body by acting as a heart, so that Selwa Hossain survives normally. She is the second person in the history of the world to live with an artificial heart. The total cost of her treatment is 96 lakh rupees. More research ahead will be able to further reduce the cost of this treatment. Then maybe many more like her will be seen surviving with this treatment.She can't survive without that bag on her back. Doctors say Selwa can survive a maximum of 90 seconds if there is a problem with the bag machine. And how good we are, how healthy we are, yet how much we complain about life! A haunted palace My dreams are usually very strange. But I remember all my dreams more or less. Sometimes I dream even when I sleep for a short time. Sometimes I dream many dreams together. Then I remember some dreams and forget the rest. But I still remember many funny dreams. Today I will share a dream with you. I dreamed that my father had bought a big and amazing palace. People say that there used to be a royal family in this palace. But at one time all the members of the royal family died except the king's only one daughter. At that time many people wanted to buy this palace from the princess.But as long as the princess was alive, she did not allow the palace to be sold. One day, the princess also died. Then my father bought the palace. On the occasion of the purchase of the palace, my father invited all the people of the state, including relatives. Everyone was very happy. The children started playing in the playground of that palace. The adults started talking among themselves. At that time the people of the kingdom discussed that the soul of the princess was still roaming in this palace and guarding the palace. Anyway, when everyone finished eating, they all left for their home. Then my father came and said that a terrible incident had taken place. As people were leaving the big gate of the palace, 10 people died in the crowd. We were very scared when we heard the incident.But there is no fear in the eyes of my father. Then at night I talked to my father and my mother. I told my father," I am very scared if the princess really comes to kill us as a ghost! "Dad said, "There is nothing to be afraid of. There is no such thing as a ghost in the world. "Suddenly there was a scream from outside. My mother then said," Who is screaming? If you have the courage, come forward. "My mother then came out of the room to see who was screaming.Suddenly, I felt as if someone had put her hand on my shoulder.The touch of her hand is a little hot. Whenever I looked to the side, I saw a woman covering her whole body with a black cloth. I was scared. I should have screamed, but instead the woman saw me and ran out of the room with screaming. Then I woke up. Surprisingly, even after waking up, I felt the touch of that hot hand on my shoulder. Then I asked everyone in the house if anyone had put their hand on my shoulder while I was sleeping.But they say, no, they didn't put their hands on my shoulders. Street Children My college friends and I often go to different places for gossiping. We would go out to visit many place after finishing of our classes . Then we would see many helpless children begging in the streets. Even many people would pick up food from the dustbin because they have no food in their house. There is no one to see these street children. But one day we saw some seniors sitting on the street and teaching some street children. We were very happy to see it. We also wanted to do something for them. One day we thought we are so tired for walking . Let's eat something. So we ordered a food called "khichuri". Just then some street children came in front of us. We thought we would feed them khichuri today instead of giving them money. A few street childs came also. Then we let them sit in the chair near to us. They ate khichuri with full satisfaction. We were very happy to be able to feed them. In fact, we all should help these street children. If we can not do anything for them though we are educated, then our education has no value. So let's all come forward to help street children. Only then we can build a beautiful world. My thought about my country Bangladesh😇 I am Meher,I live in Dhaka, the capital city of Bangladesh. My country is very beautiful. My country is surrounded by greenery.The rivers of this country are not less such as Padma, Meghna, Jamuna, Brahmaputra, Karnafuli, Buriganga etc. That is why Bangladesh is also called a riverine country. There are various tourist spots in Bangladesh. Which is really nice. Cox's Bazar sea beach is one of the most beautiful places in my country. There are also tea gardens in Sylhet, Sajek in Khagrachhari(which is the kingdom of cloud😍) and many other places of interest. Bangladesh has some historical dates, such as Independence Day on March 26, Victory Day on December 16 and Language Martyrs' Day on February 21.After 9 months of bloody war in 1971, Bangladesh gained independence in exchange for 3 million martyrs. There are six seasons in Bangladesh. Summer, rainy season, autumn,late autumn,winter,spring. The people of this country are also very sincere.The taste of traditional cake of Bangladesh is so yammy. Noodles soup Today I will tell you how to make noodles soup. However, I can not cook so well. Even, I never cook anything 😛because all the cooking is done by my mother. But once I made noodles soup. 😀So now I will share it today *****Ingredients for making noodles soup:***** 1 / Noodles 2 / Spices of noodles 3 / hot water 4 / Soya sauce 5 / Tomato sauce 6 / Green chillies 7 / Egg 8 / Lemon *****Method***** : Take 8 cups of water in a pan and wait till the water boils. When the water boils, add noodles. Then wait till the noodles are completely boiled. After 10 minutes, add 4 green chillies. After 4-5 minutes, reduce the heat of the stove and add 1 tablespoon of soya sauce. Then add 2 tablespoons of tomato sauce. After 5-6 minutes, mix the noodles spice in it. Then add an egg to the noodles. Then add one-fourth of a lemon. Some memory of a strict teacher in my school life I went to the same school from class 3 to class 10. 😓There was a strict teacher in my school. If anyone did not follow the rules, she would scold and punish her a lot. Her name was Nargis. When we used to participate in the assembly, we had some rules 😫 We should come school with two braids in the hair Must be present in the assembly at the right time. Sing the national anthem in unison. Can't move while singing the national anthem, can't talk to each other. Tall girls will always stand behind short girls. Nargis Madam used to punish them who violated these rules. Many students just moved their lips without singing the national anthem. But the teacher could understand the deceptions and punish them. Everyone was afraid of her punishment and avoided her. When she would guard us in our exam, we all in fear. We would not look at each other. No one would dare to copy and paste. And if someone cheated, she would easily understand and take the exam paper, which she would not return. Nargis Madam used to check everyone's bags on the day of her class to see if everyone had brought books on all subjects or not. She also used to clean some places by the students when she saw dirt somewhere. Half of her class time was spent on cleaning and discipline. Besides the students of the next class would be silent for fear of her. However, One day she was teaching in our side class. The students in our class were making a lot of noise. No one listened to her even though she came and forbid. Then when she went to the next class again, the class captain shouted "Stop everyone" to stop our noise.But Madam thought she had just shouted without any reason. She came from the next room and got angry and slapped the girl hard. As a result, the girl was started crying. However,She later misunderstood and apologized to the girl. She was a strict teacher but she had a good mind also.She always try to taught us how to keep clean our surroundings. That's why we are always grateful to her. Thanks for reading. Life is short. In this short life we ​​all run after success. Some succeed and some fail. But the one who l fails and tries again and again is actually a successful person. There are many examples of successful people in the world, such as **Nelson Mandela, Mark Zuckerberg, Bill Gates**, **Newton** etc. We always hear the stories of successful people but we want to avoid the stories of their failures. It is not an easy task to be successful. To be successful you need to accept a lot of sacrifices. To be successful you have to work hard. There are many people in our society who do not go the right way to be successful and take bad ways. In this way you may have success for a while. But this success will not  long lasting in your life. Soon failure will come down in your life. On the other hand, those who want to succeed with a right way, selfless attitude can achieve real success. When we want to achieve success, we fall into many dangers. But we should not stop.
