Lords of light ***Lords of light*** ***This year's Nobel Prize winners in Physics are three scientists who have played a very important role in the development of information technology. Charles Kuen Kao received half of the Nobel Prize for discoveries that laid the foundations of optical fiber technology, which is today used as the most efficient for almost all types of communication in the world. The other half of the Nobel Prize was shared by Willard Sterling Boyle and George Elwood Smith for the discovery of the digital optical sensor - CCD (Charge-Coupled Device), which today is a basic part of almost every digital camera or digital camera.*** When the winners of the Nobel Prize in Physics were announced in Stockholm, a large part of the world came to that information almost instantly. At about the speed of light, at the fastest possible speed, the message spread around the world. Text, image, and tone, through optical fibers and space, came to various devices used in homes to receive information, almost instantly. Such a fast transfer of information today is a convenience that many take for granted, so used to, that they could not easily imagine their lives without this technology. The main precondition for the development of communications as we know it today was optical fibers, which Charles Kao invented about forty years ago. After only a few years, Willard Boyle and George Smith brought a revolution in the field of photography, because thanks to their discovery, the film is no longer needed by cameras, because images can be captured using an electronic chip (CCD) that they designed. This discovery paved the way for the digital transmission of images, which are now circulating en masse on the Internet, through optical cables around the world. ***How to capture light?*** Sunlight is what makes us see the world. However, it took a long time for people to be able to control the light in such a way that they could use it to send different coded messages to multiple recipients at the same time. This technology required numerous inventions, both large and small, in order to lay the foundations of the modern information society. Fiber optic technology required the development of new glassmaking technologies. A reliable light source was also needed, which was achieved by the development of semiconductors. Finally, a huge network was needed, which would be composed of transistors, amplifiers, switches, transmitters, and receivers, as well as many other elements that would work simultaneously. The telecommunication revolution, the consequences of which we are considering, was made possible by the work of thousands of scientists and inventors from all over the world. The great world exhibition, which was held in 1889 in Paris, celebrated the centenary of the French bourgeois revolution. The famous Eiffel Tower has become one of the most famous landmarks of this exhibition. However, the extraordinary play with light that was prepared at that time was a less remembered spectacle. It was a fountain that let out jets of water illuminated by various colors. The source of this inspiration was in earlier attempts from the middle of the 19th century to make light rays that would be carried by water. These attempts have shown that when a jet of water is illuminated by sunlight, light travels through it and follows its shape. Of course, the effects of light in water or glass date back much earlier. As far back as 4,500 years ago, glass was made in Mesopotamia and Egypt. Venetian glassmakers used various combinations of lighting and shapes to create their decorations. Cut and ground glass was used to make candlesticks and chandeliers, and the mystery of the rainbow challenged the imagination of many men and women long before the laws of optics brought answers in the 17th century. However, only about 100 years separate us from the moment when these ideas matured and people started thinking about the application of trapped light rays. Rays of sunlight falling into water bend when they hit its surface because the optical refractive index of water is greater than the optical index of refraction of air. If the light beam is released from the other side, to travel from water to air, there is a possibility that it will not come out of the water at all, but will be reflected back. This phenomenon forms the basis of waveguide technology, where light is trapped inside a fiber with a higher optical refractive index relative to the environment. The beam of light that is released into the optical fiber is reflected from the glass wall and moves forward through it because the optical refractive index of the glass is higher than the optical refractive index of the air that surrounds it. Short, simple optical fibers have been used in medicine since the 1930s. Using thin glass tubes, doctors can peek into a patient's stomach or lighten a patient's tooth during surgery. However, when the fibers touched, the light would come out of each other, and they would wear out quickly. The coating of the fibers with a glass coating with a lower refractive index led to a noticeable improvement. So, by the 1960s, a big step had been made towards the development of instruments for gastroscopy and other medical needs. For long-distance communication, these optical fibers were still useless, and to make matters worse, few scientists were interested in optical light, because it was the age of electronics and radio waves. In 1956, the first transatlantic cable was laid and had a capacity of 36 simultaneous telephone calls. After that, satellites began to cover the rest of the communication needs - telephony developed dramatically, and television transmissions required even greater capacities. Compared to radio waves, infrared and visible light can carry tens of thousands of times more information, so the potential of optical light could not be further ignored. The invention of the laser in the early 1960s was a decisive step for fiber optics. The laser was a stable light source, emitting an intense and highly directed beam of light, ideal for use with thin optical fibers. The first lasers emitted infrared light and required cooling, but more practical lasers were constructed during the 1970s, which could operate at room temperatures. This was a technological breakthrough that practically gave birth to optical communication. All information could now be encoded into extremely fast flashing lights, which would represent digital ones and zeros. However, there was still no answer to the question of how such a signal could be transmitted over longer distances because, after 20 meters, only 1% of the light that entered the fiber managed to be detected. Reducing this loss of light has become a challenge for visionaries such as Charles Kao. He was born in 1933 in Shanghai and moved with his family to Hong Kong in 1948. He was educated as an electronics engineer and defended his doctoral thesis in 1965 in London. By then, he was already heavily employed at Standard Telecommunication Laboratories, where he and his colleague George Hockham carefully studied glass fibers. Their goal was for at least 1% of the light that enters the optical fiber to remain in it after 1 kilometer. In January 1966, Kao presented his conclusions. It was not the imperfections of the fiber that were the main problem, but only the glass that had to be cleaner and more perfect in order to achieve its goals. He also added that it is feasible, but with great efforts. The next goal was to make a glass of such transparency that had never been achieved before. Kao's enthusiasm has inspired other researchers to share his vision of the potential of fiber optics in the future. Glass is made from quartz, one of the most widely distributed minerals on Earth. However, in order to make the cleanest glass in the world, Kao suggested that very thin fibers be extracted from quartz melted at 2000 degrees Celsius. After 4 years, in 1971, scientists from a company that has over 100 years of experience in making glass, Corning Glass Works, from the United States, using chemical processes, produced an optical fiber 1 kilometer long. Very thin fibers, made of glass, can seem very brittle and fragile. However, when the glass is properly pulled into a thin thread, its properties change. It becomes firm, light, and flexible, which must be a prerequisite if the fiber is to be buried, pulled underwater, or bent at the corners. Unlike copper cables, glass fibers are not sensitive to thunder and cannot be affected by bad weather like radio waves, for example. It took quite some time for the Earth to connect with optical fibers. In 1988, the first optical cable was laid under the bottom of the Atlantic Ocean, between America and Europe. It was 6000 kilometers long. Today, telephone signals and data travel through a network of optical glass fibers whose total length is about 1 billion kilometers. If that length were wrapped around our planet, it would make about 250 thousand windings, and we should keep in mind that the number of optical fibers that come into use increases from minute to minute. Even in the purest glass fibers, the signal is slightly lost along the way and must be amplified if transmitted over huge distances. This problem, which previously required electronics, is now solved with optical amplifiers. This ended the unnecessary losses that occurred when the light had to be converted into electronic signals and electronic signals back into the light. Today, 95% of the light remains in the fibers after a full kilometer of travel, which is a number that should be compared to Kao's ambition to transmit only 1% of light over the same distance. Also, today it is not possible to talk about only one type of optical fiber. The choice of fibers is subject to consideration of communication needs and costs. The fibers are characterized by combinations of sizes, material characteristics, and light wavelengths. Semiconductor lasers and light grain-sized light-emitting diodes make optical cable networks stream information from entire telephony and the Internet around the world. Infrared light, with a wavelength of 1.55 micrometers, is today used for all types of long-distance communication, because the losses at that wavelength are the smallest. The capacity of optical networks continues to grow at an astonishing rate, and the transmission of thousands of gigabits per second is no longer a dream. Technological advances are moving in the direction of increasingly interactive communication, where optical cables will be more and more accessible and eventually connect even our houses with a large network. The technology already exists, and what we do with it is another common question. ***Digital eye*** Sometimes inventions seem surprising, and such certainly include the CCD sensor. Without CCD chips, the development of digital cameras and camcorders would have gone much slower. If it weren't for the CCD, we wouldn't be able to see stunning photographs of space taken by the Hubble Space Telescope or images of the red desert on our neighboring planet - Mars. The mentioned phenomena are definitely not what the inventors of CCD, Willard Boyle, and George Smith, imagined when they started their work. One September day in 1969, they laid the foundation of a photosensor model on a board in Boyle's office. At the time, they did not have in mind how to take a photograph, but their goal was to make a better electronic memory. CCD as a memory medium is now largely forgotten, but it is therefore in a very unexpected place at the time - digital cameras and cameras, as a basic component. The story of the CCD sensor is another success story of our electronic era. Just like many other electronic devices, digital photosensor, CCD, is made of silicon. The size of a postage stamp, the silicon substrate holds millions of light-sensitive photocells. Digital photographic technology uses the photoelectric effect, which was first explained by Albert Einstein, for which he received the Nobel Prize in 1921. This effect occurs when light hits a silicon substrate and emits an electron into a photocell. The released electrons are collected in cells that act as small containers. The higher the light intensity, the more electrons that fill these small containers are released. When an electrical voltage is passed through the CCD array, the contents of the container can be read - line by line, the electrons from the array exit into one type of transmitter. Thus, for example, a CCD matrix measuring 10 x 10 elements is transformed into a string of length of 100 elements. In this way, the CCD chip transforms the optical image into electronic signals that are later translated into digital ones and zeros. Each cell can be represented as one element of the image - a pixel. When the width of the CCD chip, expressed in pixels, is multiplied by the length of the same, the image capacity of the entire photosensor is obtained. Thus a CCD chip measuring 1280 x 1024 pixels has a capacity of 1.3 megapixels (1.3 million pixels). The CCD renders the image as black and white, so various filters must be used to obtain information about the colors of the light. One of the filters that leak basic colors - red, green, and blue, is placed above each photocell of the photosensitive sensor. Due to the nature of the sensitivity of the human eye, the number of green pixels is twice the number of red or green ones. Various complex filter techniques can be used for more accurate photography. When asked how Boyle and Smith came up with the idea to develop CCD during their brief brainstorming that took place some forty years ago, the answer is their employer’s internal policy at Bell Labs. Their boss encouraged them to develop a better, so-called bubble memory, which is another invention of the laboratory in which they worked. After the basic CCD model was completed, it only took a week for the technicians to make the first prototype. As a memory, CCD has long been forgotten, but it has become a center of photographic technology. Bell Labs hired the American George Smith in 1959, who recorded 30 patents during his work in this company. When he retired in 1986, he dedicated himself to his life's passion - sailing the oceans, and as a result, he sailed around our planet several times. By 1969, Willard Boyle had made several important discoveries, including discoveries related to the development of the world's first red laser. Boyle was born in a remote part of Canada, where he was educated until the age of 15, thanks to his mother, in his house. Bell Labs became his workplace in 1953, and in the 1960s, with 400,000 scientists in America, he worked on a project to send the first man to the moon on July 20, 1969. The benefits of the digital optical sensor soon became apparent. Just a year after the discovery, in 1970, Smith and Boyle were able to present a CCD in their video camera for the first time. In 1972, the American company Fairchild constructed the first photographic sensor measuring 100 x 100 pixels, which entered serial production a few years later. In 1975, Boyle and Smith constructed a digital video camera of sufficient resolution to transmit a television image. By 1981, the first camera with a built-in CCD sensor appeared on the market. Although with extremely primitive characteristics compared to modern cameras, these cameras are responsible for the beginning of digitalization of photography - digital photography was born. Five years later, in 1986, the first 1.4-megapixel sensor was made and after 9 years, in 1995, the first fully digitalized camera appeared on the market. Photo equipment manufacturers around the world quickly embraced this technology and the development of cheaper and higher quality products followed. With cameras equipped with digital sensors instead of film, the era in the history of photography, which began in 1839, when Louis Daguerre presented the photographic film as his invention, before the French Academy of Sciences, is over. When it comes to everyday photography, it turns out that digital cameras and cameras have become a commercial success. Recently, CCD sensors have been challenged by another technology - CMOS (Complementary Metal Oxide Semiconductor). It is a technology that was developed at the same time as CCD. Both technologies work on the principle of the photo effect, but while with a CCD chip, the photocells are read line by line, in the CMOS sensor each cell is read separately. CMOS consumes less energy, so batteries last longer, and its production is cheaper for a longer time. However, it must be noted that the characteristics of this chip are higher noise, as well as the loss of image quality. CMOS technology is also not sensitive enough for certain areas in which photography is applied. Today, this technology is most often used in mobile phone cameras and for some other types of photography. However, both technologies are constantly evolving to be applied for various purposes. 3 years ago, the CCD reached a chip resolution of 100 megapixels. Although image quality does not only depend on the number of pixels, reaching this resolution has taken another step in the development of digital photography. There are those who predict that the future belongs to CMOS, rather than the CCD sensor. Others, however, think that these two technologies will continue to overtake each other for a long time. Initially, no one predicted that the CCD would become the basic tool of modern astronomy. However, thanks to digital technology, the wide-angle camera of the Hubble Space Telescope is able to send beautiful photos of space to Earth. The sensor of this camera initially consisted of only 0.64 megapixels, but later, when 4 such sensors were interconnected, the camera gave photos with a resolution of 2.56 megapixels. This was a big deal in the 1980s when the telescope was designed. Today, the Kepler satellite is equipped with a 95-megapixel mosaic sensor, and scientists hope to discover Earth-like planets orbiting other stars. Earlier, astronomers realized the benefits of a digital optical sensor. It covers a huge part of the electromagnetic spectrum - from X-rays to infrared light and is thousands of times more sensitive than photographic film. Out of 100 photons, a CCD is capable of capturing up to 90, where a photographic emulsion or human eye would barely capture 1. In just a few seconds, the CCD collects light coming from distant celestial objects, which previously took several hours of photographic exposure. In 1974, the digital photographic sensor was used for the first time to photograph the Moon, and after that, astronomers accepted this technology with lightning speed. A digital camera with a resolution of 320 x 512 pixels was mounted on one of the telescopes of the American National Observatory Kitt Peak in Arizona, as early as 1979. Today, whenever photography, video, or television is used somewhere, digital optical sensors are certainly involved in the process. They are also used for surveillance purposes, both on Earth and in space. CCD is also used in medicine to record internal organs for the purpose of diagnosing the disease. The digital optical sensor has become a widely used technology, both in science and in everyday life, both on the ocean floor and in space. Originally, Rona wanted to design a campaign based on the fact that they are a company that recycles paints, which resulted in placing their billboards just below Apple's billboards. Their billboard contained paint buckets placed just below Apple's products so that the paint "leaked" directly to them. The sign that dominated the billboard was, "We recycle paint residue." ***This initiative was very unique and innovative. The result is about 2 million liters of recycled paint.*** We must note that this example of ambush marketing is not typical. For years, popular culture (movies, comics) has introduced us to various depictions and personifications of *****VI*****, and developments in the film industry have only made it easier to show them on the big screen. Thus, science fiction can and does try to touch as much as possible the issues that have been the topic of debate for decades, the possibility of creating an artificial mind, a machine that works and, more importantly, reacts as a human being, that is, an intellectual being. The ethics of this endeavor have been the subject of controversy since it became an idea. *****History***** A few years ago, the secret of the British Secret Service was revealed, that during the Second World War they managed to break the German *****ENIGMA***** and thus shorten the war by at least two years and win. They achieved this primarily with the help of Alan Turing, a British mathematician, and decoder who found a way to make a computer that would solve a given problem, in this case, coded messages of the German navy by behaving like him and examining combinations until an exact solution is found. *****ENIGMA***** was considered invincible until this discovery. It is Turing who is considered the father of artificial intelligence. Of course, making supercomputers in the middle of the *****20th***** century was unimaginably difficult, considering that it was an incredibly "expensive sport". Only the most prestigious universities with the best computer labs had any chance because of funding. In the '60s and early *****'70s***** of the *****XX***** century, this field is in a great development. The machines could store much more data, which was a prerequisite for *****VI.***** Scientists themselves have become more experienced, and they have better defined problem-solving algorithms and better determined which of the algorithms to apply to a particular problem. What particularly interested the financiers, at that time the government organization, was the recording and translation of the direct speech. Needless to say, this application was of immense value at the time, the Cold War, and the golden age of espionage. After advances related to the algorithms themselves and the development of the machine in the form of memory and speed, as well as advances related to certain fields of application, *****VI***** stopped developing. Although with predictions that they are close to the development of the artificial mind, however, there was no end result. In the absence of government investment, the field of artificial intelligence flourished at the end of the last century. So Elon Musk, one of the most successful entrepreneurs, and Stephen Hawking, a world-renowned astrophysicist, agree that if there is no control over the development of ***VI***, there can be catastrophic consequences for humanity, to the limits we see in sci-fi movies like Terminator. On the other hand, Bill Gates and Mark Zuckerberg disagree with their findings and claim that this opinion is paranoid. They see infinite potential and support every kind of development of artificial intelligence. When the day of surgery finally came, because you could literally divide the tension into two outcomes, and the positive one seemed so far away at that moment. The next few days of the procedure were equally a nightmare, because we waited day after day to be told what was happening with the embryos. Every call we received created more and more nervousness. After all, only one embryo was viable, and thankfully, it was as perfect as it could be in that situation. We decided to do the transfer on the third day instead of taking a risk and waiting for the fifth. On the fifth day, it could freeze and be sent for genetic testing, so we were aware that we were taking a risk. Still, after all, the risk paid off. We were not wrong. *****Good news...***** Two weeks later, I was in Brooklyn looking for a potential location for a restaurant, when I received a message from my wife. It was a picture of two positive pregnancy tests. I will never forget that feeling that went through my body because my dream of becoming a father has finally come true. We certainly had a long way to go, with more incidental scars, but we stuck together and fought with all our might for the family. Our son Leo was a real miracle, who taught us a lot about life, and so young. We didn't think that we would start creating a family in this way, but life doesn't always go the way we thought. *****Happy ending...***** Now, only two years after the entire *****WTO***** procedure, we are expecting another son. This baby is also a miracle of in vitro fertilization, which went in the same way as the first one - with one quality embryo and transfer on the third day. I realized that we are incredibly happy that we managed to have a baby for the second time. This story has a happy ending, but there were moments in my life when I didn’t think I would have a chance to tell it. My story is intended for all future fathers. I want you to know that we are all together in this. We should not feel ashamed or insecure about our problem. The essence of all IVF procedures is to induce ovulation and deliver the prepared semen to greater or lesser proximity of the ovum in the body (AIH) or outside the woman's body (IVF). *****ARTIFICIAL INSEMINATION - AIH OR IUI***** Insemination is a simple intervention and lasts from 5-10 minutes and in most cases is completely painless. ***1-1.5 ml*** of previously prepared semen is inserted into the uterus with a special catheter. After insemination, it is recommended that the woman continue to lie down for some time. Two weeks after insemination, it is necessary to do a blood test in order to establish a pregnancy. Insemination is the process of taking the husband's semen (sperm) and its artificial introduction into: *****cervical canal - cervical insemination***** • transcervical uterine cavity - intrauterine insemination (IUI) • fallopian tube transcervical - intratubal insemination ***Insemination can be one of the types of infertility treatment for couples who have been diagnosed with idiopathic infertility, milder forms of male infertility, and ovulation problems.*** *****Insemination can be reported***** • in a spontaneous (natural) cycle at the time of expected ovulation (by determining luteinizing hormone levels) • with the stimulation of ovulation at the time of expected ovulation (by determining the level of luteinizing hormone) or • with the stimulation of ovulation at the time of planned ovulation (application of medications that allow us to program the time of ovulation), but always with prior preparation of semen in order to achieve maximum fertilizing potential of the spouse Timing in insemination is very important because insemination is done either immediately before or immediately after ovulation. There are several methods available to doctors to determine the exact date of insemination, such as ultrasound examinations and determination of luteinizing hormone levels. If the cause of infertility is the absence of ovulation, insemination can be tried in several cycles, however, most couples who have a problem with pregnancy turn to in vitro fertilization after 3 unsuccessful insemination procedures. *****IVF FERTILIZATION - IVF AND ICSI***** In vitro fertilization is performed as an infertility therapy only when there is no other simpler way to treat infertility and this method is the crown of infertility treatment and for many couples the only way to parenthood. *****We must note that the pregnancy caused by in vitro fertilization is exactly the same as any other pregnancy that occurred in the usual way.***** Any complications in such a pregnancy can in no way be attributed to in vitro fertilization. There are two basic types of IVF procedures - the classic IVF IVF method and ICSI micro fertilization. The decision of which of these two procedures will be applied in the treatment of infertility depending on the number and quality of eggs, as well as the quality of sperm (their number, morphology, and motility). Regardless of which laboratory procedure of merging the egg and sperm is performed, the preparatory phase, which is the procedure of controlled ovarian stimulation, is the same. *****Preparatory phase - Stimulation of ovulation***** Oocytes develop inside the fluid-filled cavities in the ovaries, called follicles, which enlarge as the egg matures. Controlled ovulation hyperstimulation is a procedure used in in vitro fertilization procedures with the goal of stimulating the ovaries to produce as many eggs as possible, compared to only one as is the case in natural cycles. More eggs, and in connection with that, more embryos, provide the possibility of choosing the highest quality embryos for embryo transfer. The effect of the therapy taken in order to create a larger number of eggs is monitored on ultrasound examinations (folliculometry) and on the basis of the levels of the hormone estradiol. Also, ultrasound monitors the thickness and maturity of the uterine mucosa. *****Follicle puncture and oocyte aspiration***** In medical terminology, puncture of cavities is referred to as follicle puncture, and egg suction as an aspiration of oocytes. This intervention is performed under general anesthesia, vaginally, under ultrasound control. After the intervention, the biologist immediately examines the submitted material under a microscope and announces the number of obtained eggs. *****In vitro fertilization - Laboratory phase - IVF***** After collecting the eggs, they are transferred to the laboratory, where they are evaluated and prepared for mixing with sperm. In the case of the classic ***IVF*** method, ***IVF is*** "offered" a larger number of quality sperm in special conditions. When eggs are inseminated, they are examined approximately ***14-18 hours*** afterward to determine if fertilization has occurred, and ***24 hours*** later to verify that adequate division has occurred. ***In vitro fertilization - Laboratory phase*** - *****MICROFERTILIZATION - ICSI***** In vitro fertilization by intracytoplasmic sperm injection - Microfertilization ***(ICSI)*** is an in vitro fertilization procedure used when we want to increase the chances of a couple involved in the procedure, which has a minimal probability of fertilization occurring naturally ***(classical IVF method).*** Clinical indications for this in vitro fertilization procedure include: ***Male sterility factor*** • decreased sperm count • poor sperm motility • poor sperm quality • sperm that lack the ability to penetrate the egg ***Immune factor*** • idiopathic sterility (of unknown, more precisely, undiscovered cause) • In previous cycles of IVF, there was no fertilization, or it was significantly reduced, ie. only a very small number of eggs were fertilized. Under normal circumstances, the egg is surrounded by clusters of cells that must all be removed, and the egg cleaned before the IVF technique is approached. As with the classic IVF method, sperm is obtained from a partner and special laboratory techniques prepare for further procedure. In the most severe cases of the complete absence of sperm in the ejaculate (azoospermia), a testicular aspiration biopsy ***(TESA)*** can be performed, ie a real surgical testicular biopsy (***TESE***). When both sperm and eggs are collected, a process occurs in which the biologist-embryologist injects one sperm into the egg. After this procedure, the egg is examined approximately ***14-16 hours*** afterward to determine if fertilization has occurred, and ***24 hours*** later to verify that adequate division has occurred. *****HATCHING***** Hatching is a new method that can improve the chances of IVF success in certain situations. By observing the embryos in the laboratory, it was determined that those embryos that are surrounded by a thin natural shell have a higher percentage of implants in the uterine mucosa. This is how the idea of micromanipulation called hatching came about. ***On what principles are hatching based?*** The embryo that is inserted into the uterus by the usual embryo transfer technique is wrapped in a sheath. In order for it to be implanted in the mucous membrane, the embryo must come out of its shell, that is, it must "hatch" like a bird from an egg. Nature sometimes makes this envelope too thick or firm, so that cells that are in the accelerated division do not have enough energy to break through, and rejection occurs. The solution to this problem is hatching. It thins and pierces the sheath and facilitates the release of the embryo and its subsequent implantation in the mucous membrane. The methods by which this is achieved can be chemical, mechanical, and laser. Lately, the laser has been accepted as the most reliable and is performed in all large biological laboratories. The reliability of the laser in relation to other methods is, above all, the precision of execution, but also the speed. Hatching is performed immediately before embryo transfer. It is used both on fresh, but also on embryos that have been frozen. Statistics show that less than 1% of quality embryos during this micromanipulation suffer damage that makes them incapable of implantation. However, that is the main reason why hatching is tactically given up in those situations when there is only one embryo. In practice, hatching can be used in all cases of in vitro fertilization. However, experience has shown that this method is especially useful in the following situations: • later reproductive years, ie in patients older than ***37 years*** • patients who have elevated basal levels of follicle-stimulating hormone ***(FSH)***, which indicates ovarian depletion • situations where embryonic cells divide more slowly or are fragmented • in cases where the ova have an evidently thicker and/or firmer sheath (pellucid zone) • patients who have had several unsuccessful ***IVF*** attempts (three or more embryo transfers that did not end in conception) The results so far show that in all these situations, hatching can increase the chance of pregnancy by ***20%.*** *****EMBRYO TRANSFER***** Embryo transfer, as the end of the in vitro fertilization procedure, can be done as early as thirty hours after the intervention. This means that the return of the embryo takes place from one and a half days to three days after the puncture of the follicles and the aspiration of the eggs. In certain situations, this period is extended up to five days, until the embryo reaches the "blastocyst" stage. It is important to emphasize that blastocyst culture is not possible for all couples. Not recommended for patients with six or fewer fertilized eggs. The process of inserting the embryo into the uterus does not take long, and it is not painful. Before the embryo transfer itself, in "Jevremova", patients have the opportunity to see their embryos that will be returned to them. After the embryo transfer, a short rest in our apartments is advised, and after that, the patients are discharged home. ***Implant outcome*** Fifteen days after the embryo transfer, a test should be done to prove early pregnancy. This sensitive test is called beta-HCG and is done from the blood.

Industrial robots Various ways of systematizing industrial manipulation robots can be found in the literature. Very often, the criteria used are based on their technical characteristics and are fully valid only for a certain period of time. A division based on the type of mechanical structure of the minimum robot configuration is adopted here, since it is one of the very important characteristics, and which is relatively unchanged over time. The basic configurations of industrial robots will be presented below. ***Robot of anthropomorphic configuration:*** In most robotic configurations, including this one, the first degree of freedom (calculated from the substrate) is rotational and the axis of the first joint is vertical. This joint ensures the rotation of the complete robot around the vertical axis. The axes of the second and third joints are mutually parallel, horizontal, and perpendicular to the axis of the first of the second and third joints, it is ensured that the top of the minimum configuration is positioned at an arbitrary point in the vertical plane containing the axis of the first joint. A sketch of the kinematic structure and a photograph of a robot of anthropomorphic configuration are shown in the figures. The mechanical structure of the robot of anthropomorphic configuration is adjusted to the structure of the human hand, so the name of the upper arm is often used for the second segment of the minimal configuration, and for the third forearm. The basic characteristic of a robot of anthropomorphic configuration is that it can cross an obstacle that gets in its way. If we look at the kinematic scheme of this configuration, it is clear that starting e.g. of the third joint affects the intensity of the moment around the axis of the second joint even if this joint does not move. This effect is called coupling. Therefore, it is said that if the movement of one joint significantly affects the driving moments (or driving forces if the joints are translational) of the other joints, the observed configuration has a significant coupling between the joints. Robots of anthropomorphic configuration are characterized by the fact that they have a large coupling between the joints ***Robot of spherical (polar) configuration:*** In the spherical configuration as well, the axis of the first joint is directed vertically upwards, while the axis of the second joint is horizontal and perpendicular to the axis of the first joint. The third joint is translational. Therefore, the spherical configuration of the species and the arrangement of the first two joints are the same as in the anthropomorphic one. Kinematic scheme of spherical conf duration is shown in the figure. As before, the second and third joints allow the position of the top of the minimum configuration to be positioned at any point in the vertical plane, while rotating the vertical plane around the axis of the first joint, the robot tip can be positioned anywhere within the workspace. This configuration is called spherical because of the shape of the workspace that is part of the sphere, and polar because the angle of rotation of the second joint and the elongation of the third joint corresponds to the polar coordinates. It should be noted that, since the third joint is translational, the approach to the workplace should be unobstructed since a robot of this configuration cannot bypass them. ***Cylindrical robot configuration:*** The minimum configuration of a robot of cylindrical configuration has one rotary and two translational joints. In this, as in the previous configurations, the first segkinematska_sema_cilindricna.jpgment represents a rotating column around the vertical cylindric, so the first joint is rotary and placed in the same way as in the previous configurations. The second and third joints are translational (linear). The axis of the second joint is vertical, which means that its movement raises or lowers the complete structure that is further from the joint, towards the top of the robot. The axis of the third joint is horizontal so that by moving it, the gripper moves or moves away from the vertical column of the robot. This configuration is called cylindrical according to the shape of the workspace that is part of the cylinder. Robots of this configuration have very little coupling between the joints. ***Robot care configuration:*** ***SCARA*** configuration robots ***(Selective Compliance Assembly Robot Arm)*** have two rotary and one translator joint. Two mutually parallel rotary joints with vertical axes of rotation are placed on the column base so that both segments move in a horizontal plane. also vertical as seen in the figure. The grip joint usually has only one degree of freedom and that rotation around the vertical axis. Therefore, SCARA configuration robots typically have only four degrees of freedom. By moving the rotary joints, the positioning of the translator's joint is performed in the desired point of the horizontal plane, and then by lowering the translator's joint, the gripper is brought to the desired position within the working space. *The SCARA* configuration has very little coupling between the joints since the gravitational load is fully received by the mechanical structure of the rotary joints itself. Robots of the SCARA configuration were first realized in Japan in 1972, and are mainly intended for the realization of assembly tasks. They are characterized by high positioning accuracy and speed of work, as well as relatively high load capacity. The biggest disadvantage of the S CARA robot comes from its construction - it is placed high and occupies a large part of the space above the space in which the work tasks are performed so that space must be free. ***Robot of Cartesian (rectangular) configuration:*** The minimal configuration of these robots, as can be seen from the figure, consists of three translational joints whose axes are parallel to the axes of Cartesian rectangular coordinate system kinematic_sema_decartous. Hence the name of this configuration you can find several other names: rectangular, Cartesian ***(Cartesian)***, or portal ***(gantry)*** configuration if the robot is placed on a stand that rises above the workspace to which it accesses from above). Considering the type and arrangement of joints in robots of this configuration, the contribution of movement in each of the joints to the movement of the gripper is clear, which makes programming and even manual guidance much easier. The working space is a parallelepiped whose dimensions depend on the range of motion of each joint. Robots of this configuration have very little coupling between the joints. In order to reduce the occupied space in the drive, robots of this configuration are very often placed on a stand that rises above the working machine (portal or gantry robot). Of course, in such cases, access to the working space of the working machine must be free from above. The dimensions of the stand can be such that the robot can service several machines. ***Parallel robots:*** All the configurations described so far had the basic form of a kinematic chain, and only the type and disposition of the joints differed. In that sense, parallel robots are fundamentally different comparably instructively, but we believe that parallel needs to be mentioned here. They represent two platforms (in the nominal position these platforms are parallel) which are connected by segments of variable length. One platform is considered a base or base (upper platform), and the other is a working one whose position and orientation are controlled and there is a handle on it. By changing the distance between the corresponding joints on the base and work platform, the position and orientation of the work platform on which the gripper or tool that the robot carries is changed. The change in the distance between the joints on the base and working platform can be realized either by segments of variable length (telescopic segments with translational joints) or as two-segment elements with rotating joints as shown in the figure. Robots of this configuration are characterized by exceptional agility (speed of movement when moving from one position to another), high rigidity of position holding and relatively low mass in relation to the load capacity since each segment (arm) carries only one-third of the load, unlike kinematic chain type robots where each segment suffers the entire load.

Why is he so important?*** The function of the immune system is to protect the organism from all factors that could damage our health. With a severely damaged immune system, no one can survive despite the therapy with the most effective antibiotics. The immune system is activated every time any foreign body or organism is found in the human body. These are most often microorganisms (bacteria, viruses, fungi), as well as pollution particles, or toxins, and we call such factors antigens. There are millions of antigens that can have even slight differences between them, which the immune system recognizes and against which it starts a reaction called the immune response. A very important task of this system is to remember foreign substances and distinguish them from those that belong to the human body. ***What does the immune system consist of?*** The organs of the immune system are lymph nodes, spleen (located in the upper left part of the abdomen), breast (thymus), tonsils, and adenoids. All of them are interconnected by a system of lymph vessels through which lymph flows - a colorless fluid that contains lymphocytes (a type of white blood cell) that play a major role in the body's immune response. The lymph flows from the blood into the peripheral lymphatic capillaries, which then lead it to the larger lymph vessels, and eventually, the large lymph vessel flows back into the venous system. ***Lymph nodes*** On its way from the lymphatic capillaries to the venous circulation, the lymph passes through the lymph nodes that act as filters in which harmful substances are neutralized. The lymph nodes are spherical in shape and consist of lymph tissue. They are located in different places in the human body: under the armpits, on the groin, in the abdomen, chest, on the back of the knee, on the neck, back, and under the jaw. Pea grain sizes are normal, but they increase during infection. This happens due to the accumulation of antibodies, and with the goal of destroying the antigen. Lymph nodes are always enlarged at the site of antigen entry, so for example, lymph nodes in the neck are enlarged in the case of mononucleosis, or in the groin in the case of genitourinary infections. This means that the immune response is strongest at the place of the beginning of the infection, with the aim of preventing its further spread. ***Thymus*** It is located on the back of the sternum. In this gland, the process of maturation of one type of lymphocytes marked as T lymphocytes takes place. ***Spleen*** It is located on the upper left side of the abdomen, just below the diaphragm. It has a role similar to lymph nodes, except that it breaks down red blood cells at the end of their life. Tonsils and adenoids The palatine tonsils are located symmetrically at the bottom of the palate, and on both sides of the pharynx, while the adenoid organs are composed of lymph tissue located in the upper part of the pharynx. The palatine tonsils, adenoids, and lingual tonsils, which are located at the root of the tongue, together form the so-called lymphatic ring, responsible for the defense of the upper respiratory tract and digestive tract. Their task is to keep microorganisms and other harmful particles at the very entrance into the organism, as well as to trigger an immune response in the sense of secreting antibodies. This function is active only in the first years of life. ***Lymphocytes*** Lymphocytes play the most important role in maintaining human health. It is a type of white blood cell. We distinguish two types of lymphocytes, namely T and B lymphocytes. Each of them has its own task. T lymphocytes have the role of recognizing and distinguishing foreign substances from those that belong to the organism and to destroy them, while B lymphocytes regulate the production of antibodies. Lymphocyte synthesis begins in the bone marrow from progenitor cells that later mature into T lymphocytes in the thymus while B lymphocyte maturation ends in the bone marrow. Mature T and B lymphocytes then pass into the blood, and from the blood into peripheral lymphatic organs such as lymph nodes, spleen, and others. In the blood and lymphatic organs, they are always present and ready to react to any foreign antigen. ***Types of the immune response*** There are two types of immunity, and they are innate and acquired immunity. Innate immunity consists of a number of non-specific defense mechanisms, and they are active even before exposure to an antigen. Innate immunity means, for example, the acidic pH value of the vaginal mucosa in order to prevent the development of pathogenic microorganisms in that area, or the cilia of the ciliary cells of the bronchial mucosa that retain harmful particles and push them out of the body. This type of immunity is present from birth and represents the first line of defense. It acts immediately within 96 hours, and has no immune "memory." Acquired or specific immunity develops gradually after birth and implies that there is a specific immune response for each antigen. It is highly effective, has an immune "memory", which means that it has the ability to remember every previous contact with a particular antigen, ie to recognize foreign matter even many years after the first contact and to trigger an immune response to destroy it. It has a longer reaction time, from 96 hours and up. Lymphocytes and antibodies are an integral part of acquired immunity. The immune response can be humoral (directed against bacteria) and cellular (against viruses, malignant tumors). ***Humoral immune response*** It is based on the production of specific antibodies against a specific antigen. Antibodies are substances that are also called immunoglobulins and are secreted by B lymphocytes. After the penetration of the antigen into the body and its recognition, B lymphocytes are transformed into plasma cells that secrete antibodies against one or more antigens that caused the immune reaction. There are five types of antibodies in the body, ie. immunoglobulins (Ig), namely: ***A, G, M, E,*** and D. They differ from each other in chemical composition and structure. IgM and IgD are located on the cell membrane of lymphocytes and represent receptors for antigens. Thanks to them, the antigen is recognized. B lymphocytes then differentiate into plasma cells that secrete specific antibodies. The antibody binds to a foreign antigen, which causes its precipitation, inactivation (in the case of viruses), lysis (erythrocytes), phagocytosis (bacteria). How do we become immune? In an immune person who has already been in contact with a certain antigen once, the so-called B memory lymphocytes, which are activated by the re-penetration of the same antigen into the body by rapidly multiplying and turning into plasma cells that secrete large amounts of specific antibodies. That is the reason why some diseases, such as smallpox, mumps, etc. get sick only once. B memory lymphocytes and antibodies have the ability to stop the reproduction of pathogens as soon as they enter the body, ie. before the development of disease symptoms. ***Cellular immune response*** T lymphocytes play a major role. The cellular immune response takes place so that T lymphocytes come into direct contact with the antigen, even without the secretion of antibodies by B lymphocytes. The cellular immune response is important in viral and fungal infections, malignant tumors, organ transplants. T lymphocytes do not produce antibodies but secrete substances called cytokines. These molecules act as mediators between cells and destroy damaged cells, or stimulate other cells of the immune system to respond immune. T lymphocytes are responsible for recognizing foreign antigens in the body. On their surface there are receptors, similar in structure to those on B lymphocytes, but not identical. These receptors, even in the phase of embryonic development, acquire the ability to recognize what does not belong to the organism, as well as to recognize the altered cells of the organism itself. Histocompatible antigens are found on all cells in our body, even on the lymphocytes themselves. If these antigens change, which usually happens after a viral infection or the appearance of malignant cells, or if cells with a different antigen are found in the body (as in transplantation), T lymphocytes are activated and destroy such cells, while healthy ones are spared. Autoimmune diseases occur when the immune system loses the ability of immune memory and begins to recognize the cells of its own organism as foreign and to attack and destroy them. Immunological compromise is a term that means the reduced ability of the organism, ie. immune system to provide an adequate response to the invasion of pathogenic microorganisms and other harmful substances. It most often occurs in people who have been suffering from some chronic diseases for a long time or are infected with the HIV virus.

Automation of tasks using task scheduler Last week, we introduced you to a way to speed up some of the activities you perform on your computers. Today we will show you another way in which you can get rid of some of the work you do on your computer, using the Task Scheduler. Task Scheduler is a service that gives us the ability to schedule the time of automatic execution of various tasks on the computer. All you need to do is turn on your computer so that the scheduled tasks can be performed. The tips we provide in this text apply to the ***Windows 7*** operating system, although some of them may apply to other versions of Windows. ***Start Task Scheduler*** We can run the Task Scheduler in several ways. One of them would be to enter the Control Panel, and click on the Administrative Tools option, and then on the Task Scheduler. If your Control Panel items are grouped by category, click System and Security, and then click Administrative Tools (or Schedule tasks immediately). If for any reason you do not want to enter the Control Panel, there is another way to start the Task Scheduler. Click Start in the lower-left corner of the screen, and type task scheduler. ***Add a program/task to the Task Scheduler*** After we run the task scheduler, we will check if any of the scheduled tasks already exist. We do this by clicking on the Task Scheduler Library icon, which is located in the upper left corner of the screen. In case you don't see it, click on the icon below the Action option menu. Since it will also use the panel on the right side of the screen, we will open it as well, in case it is not already open. Let's say we want to automate the launch of a copy script, which we created last week (backup.bat). Why would we automate it? Well, let's say we sometimes forget to make backup copies. Even if we forget to copy, Task Scheduler certainly won't. How do we add this task to the Task Scheduler? To start, we need to click on the Create Basic Task option, which is located in the panel on the right side of the screen (or on the menu option Action, and within it Create Basic Task). A new window will open, in which we will first name our task, and give a brief description, so that, if the number of tasks increases, we know what this particular task is doing. Then we will determine how often this task will be performed (we decided to do it every day), from which day it will start, and at what time. As an action, we chose to run a program, ie a batch script that backs up our recorded positions. Since Task Scheduler does not know where our script is, we need to give it a path. In our case, the script was on the desktop, and it could be somewhere else on your computer. When we set all this, all we have to do is click on Finish. Thus, we have created a task that will be performed every day, at a precisely determined time, which is confirmed by the screen that appears after clicking Finish. Of course, this doesn't mean that we can't back up from time to time by running our own script - the task we've created doesn't conflict with our occasional double-click on the backup script. If you want to stop or disable this (or any other task), just click on the Disable option (bottom of the panel on the right), or even Delete, if you no longer want to make backups this way. To give you a better idea, here is a small extension of our example. If you tell your very obedient younger brother or sister to bring you a glass of water, they will do it. We don't have to tell them where the faucet is, where the glass is, or how long they should fill it. With computers, however, it is a little different - it is necessary to explain each step in detail. ***PROGRAMMERS*** Maybe someone, based on what we have written, will think that only those who calculate all the steps well can be engaged in programming, or they are simply geniuses. Certainly, these traits help, but they are not crucial. The most important thing is to be good at logical thinking and to think correctly about the problems you want to solve with programming.
