As soon as I went to the emergency department of Dhaka Medical College Hospital, I heard a loud noise. This is the busiest emergency department in the country.

Delwar Hossain, a small trader from Bariarhat in Mirsarai upazila, was sleeping on the floor of the corridor to get a towel.

He was seriously injured in a road accident during the month of Ramadan last year. Put two crutches standing near his head.

A few cloth bags on the floor.

Sitting on it, his wife Asma Begum said, "He went to the shop to buy goods. While fetching the goods, two vehicles from both sides hit his CNG together. He was hit in the head, legs and arms were broken."

On the day of the incident, a group of pedestrians and shopkeepers took him in a CNG to the local health complex.

Describing the horrific experience of that day, Asma Begum said, "At first there was a health complex in our village. They took it there. But they did not keep it there. They said take it to Chittagong. Her condition was critical. Her leg slipped here. When I took Chittagong, all the clothes in the car were covered in blood. "

He said, "There was an accident after Asr. I got medical treatment at two o'clock at night."

In other words, the journey of such a horribly injured person first to the accident site, then to the local health complex, the last road to Chittagong Medical College Hospital. In all, it took about 9 to 10 hours to start the right medical service.

Now Asma Begum, Delwar Hossain and their families are living in the hospital.

There are still many problems with the legs. Has undergone surgery several times. Will have to do it again.

Delwar Hossain, who was disabled, at least survived. Asma Begum is getting a chance to be by her husband's side.

Asma Begum spends her days serving her accident victim husband.

But a large number of injured people are the victims of road accidents in Bangladesh.

Razia Begum of Chowhara in Comilla was sitting next to the morgue of Dhaka Medical College.

A week ago, his rickshaw puller father was seriously injured in an accident. He died on the way to Dhaka Medical College on Wednesday.

That's how he was brought to the hospital. Now his body is being taken back to the village.

The only difference between the two similar stories is that Razia Begum's father did not survive.

On an average, three-quarters of road accident victims in Bangladesh die on the way to hospital. Or 73 percent of deaths in road accidents occur on the way to the hospital.

What is the reason for such a high number of deaths in Bangladesh?

CIPRB Executive Director. AKM Fazlur Rahman says in the aftermath of the accident, the main reason is the absence of primary emergency services before reaching the hospital.

After a road accident in Bangladesh, it often takes a long time to get treatment.

He said, "Emergency medical services are what we call the developed world. If an accident happens, it is first reported to a call center. Then trained manpower is sent. After rescuing the team, the hospital will have a system where the accident happened." "People are ready in the emergency department of the hospital. They are treated in the emergency room and then admitted indoors if needed. But we have not developed these three issues together here."

In the developed world, he says, there are separate units called paramedics.

These special staff are trained to give first aid in the event of an accident. Dr. Rahman thinks that it is very important to create such a unit.

What is the picture of emergency primary service in Bangladesh?

For some time now, there has been an emergency call center number 999 in Bangladesh, but there is no emergency medical service.

Dr. Rahman says that even if the police come to the spot and rescue him and take him to the hospital or provide legal help, the police cannot provide treatment.

This is a very important time to waste. In many cases, the survival of the injured person depends on the minutes of the accident.

It is seen that ambulance is not available if needed. Although it was found again, only the driver and assistant of the car are with him.

The ambulance does not have any equipment as there is no one qualified to give emergency treatment.

Dr. speaks of the absence of primary emergency services in Bangladesh. AKM Fazlur Rahman.

In Bangladesh, people are usually the first helpers in road accidents.

They are also not trained to take the seriously injured to the hospital properly.

Pulling causes more damage to the patient.

Giving a description, the Secretary General of an organization called Bangladesh Society of Emergency Medicine. Ragib Manzoor. "Suppose there is a car accident. One person is badly injured in the spine. The locals do not know how to move him. The way they are pulled out of the car and taken to the hospital shows that the injured person's spine is more damaged," he said.

Many times, pedestrians do not want to come to the aid of seriously injured people for fear of getting involved in legal complications including narrating the incident to the police, said Dr. Rahman.

He says. "This requires a Good Samaritan Law. The law will give these people an advantage so that they can help someone not to get caught up in any law."
