I've had contracts at pharmaceutical companies. (I was a COBOL programmer.) Some of these companies were pretty good, and I thought did a decent job trying to put out a safe product. Others? Not so much. Like nearly everything else it comes down to who was in charge. Good leaders head up good organizations. Piece-of-crap leaders head up piece-of-crap organizations.
I happen to think these are deliberate mistake. There is too much stuff to be just coincidence.
