To run an impact wrench, it is a big question what size of air compressor do I need for an impact wrench. After discussing which factors we should consider, let us discover the right one for you which suits your work. We will also try to recommend the best air compressor needed to run your impact wrench effectively. **What is an impact wrench?** An impact wrench may be a wrench machine designed to deliver high torque output with minimum effort, by storing energy during a rotating mass, then delivering it suddenly to the output. **Factors that we should think about before choosing an air compressor** An air compressor is a widely used tool both in commercial and personal matters. This is a pneumatic device that converts power into potential energy which is stored in pressurized air. Here two factors that determine the effectiveness of an air compressor. These are CFM and PSI. CFM means Cubic Feet per Minute. It may be defined as the measurement of the velocity at which air flows into or out of space. These are only worthy for construction use. One inch impact wrench is not seen typically. As it is a big size, it needs a heavier investment. One can go for an air compressor between 9-10 CFM range and that tool should be capable of creating at least 90-125 PSI air pressure. **What tank size of air compressor do I need for an impact wrench?** However, talking about air pressure is not enough to conclude. We should consider the size of the gallon to choose the best air compressor. Air compressors differ by their Gallon sizes. Talking about the minimum, a 6-gallon air compressor with 3.5 -5 CFM is necessary to operate an impact wrench. But this is not sufficient for day to day work, let alone professional work. To clarify, the larger the size of the gallon, the better the performance of the air compressor. But the cost matters. A 3-gallon air compressor cannot operate an impact wrench at all. Suppose, for personal use, one can recommend a 10-gallon size air compressor. Journey inside disease(Diabetes Mellitus-2) Hello learners This is my second day with you to travel inside a disease **Diabetes Mellitus**. So Lets dive in.... **Diagnosis of diabetes mellitus** I hope readers will find easy to explain biochemical results and will be able to diagnose diabetes mellitus on their own. *There are 3 ways or 3 diameters to confirm the diagnosis of Diabetes Mellitus.* The first one- 1. **Fasting blood sugar level or (FBS** ) - more than or equal to 7 mmol/L Or - more than or equal to 126 mg/dL The second diameter which we can use is 2. **Oral Glucose Tolerance Test or ( OGTT)** - more than is equal to 11.1 mmol/ L - more than or equal to 200 mg/dL And the last one is 3. **HbA1C( Glycosylated hemoglobin A 1C**) - more than or equal to 6.5% Do you know the normal levels of blood sugar level in your body. Incase answer is no lets proceed. Very very important to remember is **Normal value of blood sugar level:** 1. Fasting blood sugar level- 3.5- 6.1 mmol/ L 2. Random blood glucose level- 3.5 - less than 7.8 mmol/ L 3. Glycosylated hemoglobin A 1 c ( HbA1C) is less than 5.7 mg/dL Now you can check your own blood sugar level in nearby diagnostic center and be sure your blood sugar level is within the normal range. Because increased blood sugar level have various bad effects on your health. Which we gonna discuss later. Previously I talked about a test named *****OGTT*****. Many of you may find this term new and today I gonna dive inside this test. ****Oral Glucose Tolerance Test**** What do you think when will your doctor advise you for OGTT. Well, theres some conditions behind them. Lets understand **Indication of OGTT:** 1. If fasting blood suvar more than 6.1 mmol/L to less than 7 mmol/ L we should go for OGTT. 2. If random blood sugar level is 7.8 mmol/ L to 11mmol/L. 3. To diagnose pre diabetes OGTT is done. Pre diabetic condition: a. IFG( Impaired Fasting Glucose) b. IGT ( Impaired Glucose Tolerance) 4. Also OGTT is done to diagnose gestational diabetes mellitus. Like many other biochemical tests OGTT also has some prerequisites too.. **Prerequisites of OGTT**: But before doing Oral glucose tolerance test we should consider the following; 1. Discontinuation of medication known to effect glucose tolerance. ( Note: we must stop using drugs that hamper glucose metabolism 7 days before ogtt is done These drugs include: thiazides, corticosteroids, OCP-oral contraceptive peal. ) 2. Test should be performed in the morning after 3 days of unrestricted diet and unrestricted activity ( unrestricted diet means having 150 gm of carbohydrate per day) 3. Test should be performed after 10 hours of fasting. Guys, this is an important matter to keep in mind. 4. Incase of hospitalized patient or hospital admitted patient this test cannot be done. Readers here I wanna ask you a question can you guess why? Why OGTT cant be done for hospital admitted patients. 5. Test should be performed without smoking cigarettes. ***Readers, please dont feel bored. Just follow me and we gonna discover deeper inside of OGTT.*** Now we have gained idea about why when and before what we should have our ogtt done . Lets have a look about the roadmap of this test. **Procedure of OGTT:** 1. Test should begin between 7 am to 9 am. 2. 10 hours fasting is mandatory. 3. Estimation of fasting blood sugar level. 4. Recommended glucose load is 75 gram dissolved in 300 ml of water and ingested over 5 minutes. 5. Two hours after oral glucose intake again blood sample is collected and estimation of blood glucose level. Simple, our oral glucose tolerance test is over. Now lets wait for the result. Is it wise to have an icecream when we feel hot??? Hello readcashers, Lets learn together. Today I want to ask you an interesting question. What do you think ? Is Ice cream for hot weather countries like middle east (saudi arab, qatar, dubai) or for cold weather countries like westerns ( newzealand, UK, spain) . Do you think people of hot climate should love ice cream ? Lets come to the answer. If you thought that it is wise to have ice cream when you are feeling hot then you are doing a mistake. May be you are surprised. Let me collaborate. To understand why am I saying that you should not have the taste of ice cream when you are feeling hot, you have to think a little bit in past. We should go through the history of ice cream. In cold countries, people used to eat cream in their meals. Creams contain lipid and fatty substances, which when digested and assimilated, yield high energy and produces heat. So to increase body temperature they were eating creams. As they have plenty of ice they used to mix creams with ice to have a better test. From that icecream came. But now we dont go through the whole matter. When we feel hot, we just rush to nearby market, buy an icecream. Immediately as ice cream contains ice we feel cold and releif from hot but consequently when it is digested, it yield high energy and heat produced. Thats I wanna say you guys, we should not buy an ice cream when we are feeling hot. Guys, learning doesnt have any limit, any place, any age. Today i am going to travel inside a most common disease from which most of us or our parents, our relatives suffers a lot. May be some of you can guess which disease I wanna mean. The dangerous *****Diabetes Mellitus*****. Many people says that diabetes mellitus is the root of many many principal diseases. Again many of them wanna refer it as a morbid disease. So lets proceed in.... The diabetes mellitus **Definition of diabetes mellitus**: A group of metabolic disorders of carbohydrate metabolism in which glucose in underutilized producing hyperglycemia. Or Lets see another definition Diabetes mellitus is a heterogenous group of multifactorial, polygenic syndromes charecterized by elevation of blood glucose caused by a relative or absolute deficiency in insulin. **Note**:here ***multifactorial*** means mixture of different differnet causes genetic and environmental. Examples of some diseases are diabetes mellitus, hypertension etc. ***Polygenic*** means multiple genes involved. ***Syndromes*** means not a single problem together many problems ***Insulin deficiency*** may be absolute or relative. Absolute deficiency means decreased insulin secretion from pancreas(normally 250-300 unit secretes) normally tgese type of diabetes is type 1 diabetes. Relative deficiency means insulin resistance normally these types of diabetes is called type 2 diabetes.(usually after age 60 type 2 diabetes occurs) Now come to the classification of diabetes ****Classification of diabetes mellitus****: 1. Type-1 diabetes mellitus 2. Type-2 diabetes mellitus 3. Other specific types of diabetes mellitus 4. Gestational diabetes mellitus (Who doesnt have diabetes mellitus before but in pregnancy 1st diabetes recongnized ) Now lets collaborare each types of diabetes 1. **Type- 1 diabetes mellitus** **Causes**: a. absolute deficiency of insulin caused by autoimmune attack on beta cells of pancreas. b. Some have no evidences of autoimmunity and classified as type 1 idiopathic (cause is unknown) **Charecteristics**: - They have insulinopenia( deficiency in insulin) -seventy five percent occurs before the age of eighteen -five to ten percent of all cases of diabetes are in this group. 2. **Type-2 diabetes mellitus** 90% of all cases of diabetes are in this group. **Causes of type- 2 diabetes mellitus** 1. Insulin deficiency 2. Dysfunctional beta cells of pancreas May be many of you ask me what is isnulin resistance. Is it fighting against insulin. Ha ha ha. May be something like that. Let me clear it to you **Insulin resistance** **Definition of insulin resistance** Its is the decresed abilty of the target tissues such as liver adipose tissue and muscles to respond properly to normal or elevated circulating concentrations of insulin. Its is **charecterized** by uncontroled hepatic glucise production and decrese glucose uptake by muscle and adipose tissue. ( because of decreased insulin dependent glut 4 -Source lipincott of biochemistry) **Causes of insulin resistance**: 1. Obesity 2. Gestational diabetes mellitus 3. PCOS ( poly cystic ovary disease) 4. Growth hormone excess 3. Other types of diabetes mellitus Important Causes of them : 1. Genetic defects if beta cell function 2. Geentic defects in insulin action. 3. Disease of the exocrine pancreas. 4. Endocrinopathies A. Cushing syndrome ( steroid increses drastically ) B. Acromegaly ( increased growth hormone) C. Glucagonoma ( glucagon secreting tumor ) 5. Administrations of hormones of drugs known to induec beta cell dysfunction (dilatine, pentamadine ) or impair insulin action. ( 1. Glucocorticoids, thiazides, beta adrenergics or beta agonist ) 6. Infection 7. Uncommon forms of immune mediated diabetes. 8. Circulation-1(systemic and pulmonary circulation) Hi learners, hope all of you have a good time. Hope you will find it helpful for all. ****Circulatory systems of the body**** ****Source of figure: guyton physiology page 171**** Do you know there are 2 types of circulatory system in our body. ****1. Systemic circulation**** ****2. Pulmonary circulation**** **Systemic circulation** refers to the circulation which starts from the left ventricle and ends at right atrium. **Pulmonary circulation** refers to the circulation which starts at right ventricle of the heart and ends at left atrium. ***Systemic circulation***: here from the left ventricle through the aorta oxygenated blood is ejected and conveyed to the whole body via aorta-arteries-arterioles-capilliaries. However, then the body tissues utilizes the oxygenated blood and the blood becomes deoxygenated as it has losed its oxygen. These bloods are than collected by the venules-veins and at last via inferior vena cava, superior vena cava and thebasic veins deoxygenated bloods reaches the right atrium of the heart. From the right atrium of the heart the blood rushes to right ventricle passing through the tricuspid or semilunar valves. Now here in the right ventricle these bloods are deoxygenated so it needs to be oxygenated. For that purpose, it moves through the pulmonary artery towards the lungs (right and left) . (*****Note*****: here pulmonary arteries contains deoxygenated blood where all other body arteries contain oxygenated blood. Dont worry this is the only exception you have to remember. Again pulmonary vein is the only vein to contain oxygenated blood whereas all other veins of the body contain deoxygenated. This is also an exception. You can note it as it might be helpful for you in the next. So definition of artery can be the blood vessel which comes out from the heart . And the veins are the blood vessels which ends towards the heart Nor the definition should emphasize that which blood vessel contain oxygenated or deoxygenated. I hope everyone can understand this. About 40 milliliters of oxygen ( O2) is absorbed for each liter of blood. So in total for 5 liters of blood about 200 mililiters of oxygen is absorbed. Then these oxygenated blood reaches the left atrium of the heart via the pulmonary vein. Here pulmonary circulation ends and systemic circulation again starts from the left ventricle. Online Essay contest (I have secured second position) Hello readcashers, today I am very happy. I winned 2nd prize in online essay contest. I wanna share with you guys, my story behind that. All praise is to allah. I have winned the second prize in essay contest in category (A ) which is arranged by my medical college (faridpur medical college). This was an online essay contest arranged for ****National Mourning Day**** of my country Bangladesh. Our National Mourning day is **15 August**. There is a miserable history behind this. **Bangabandhu sheikh mujibur rahman**, ***Father of the nation***, was **killed** in 15 august, 1975 along with 18 family members by some cruel people. 10 years child sheikh rasel also cant escape from their brutual bullet. They didnt allow this little child to live. From that miserable history national mourning day of bangladesh is selected 15 august. Every year peoples of bangladesh remember their great leader in 15 august. It is a national holiday also. However many academic institutions organize different different cultural programmes, competitions such as essay contest, recitation of poem etc. For little kids they also arrange mujibs picture drawing contest as well. Like many other institution my campus, my medical college also have arranged an online **essay contest** about **bangabandhu and liberation war 1971**. It was an **online contest** because due to corona situation its not possible to arrange offline programme. There is no one in the campus 😀 There was 2 category Category A for batch F 28 and F29 Category B for batch F25, F26 and F27 As I am of batch F28 I have been participated in category A. They told me to write the essay in no more than 3500 words. However, it was not possible for me to pass the 600 words. I can hardly write about more or less 650 words. I wrote the essay on A4 paper and as their requirement I send it to my college principals email. I was totally hopeless. I didnt thought that my essay can win a contest as of I have written only a little(600 words). But now i beleive that quality is main not the quantity. None can attract judges mind by quantity without a quality. 10 days layer, at 31 august suddenly I recieved an email from principal sir. He gave me an file. I open that file and I see it was credited to me as a recognization of participation in essay contest by faridpur medical college. That certificate was so much attractive in view. I thought in mind that what ever I win or not I have earned an certificate of participation which is really amazing. I asked my friends any of them have participated or not. Swarnali replied she has. Than I thought it would take a number of months to publish the final result but yesterday was the result published. At 6 pm i get up from bed, suddenly I opened the phone and see there are plenty of messages about *****Congratulation saleh mohammed*****. I wondered what happened. Then I opened messenger group and I found this official notice of Fmc where the result of essay contest is published. All praise is to Allah that my 600 words has got the 2nd position in Category A. One of my junior who is very much talented secured the 1st position. Here my medical college said that prize giving ceremony date will be informed later. That may be seems funny but soon you will understand why I have said that. Actually today I want to discuss about how vitamin A helps in vision and how deficiency of vitamin A in diet can cause night blindness. Visual process involves vitamin A containing visual pigments present in photoreceptor cells( rods and cons ) of retina. 2. Visual pigments are basically conjugated proteins consisting of opsin(protein) and 11-cis retinal (Prosthetic group). 3. Vsiual pigments present in rods Is called rhodopsin which is most light sensitive and helps in dim light vision. *(Mnemonic: remember ro(d) =(d)im light ) 4. Visual pigments present in cons is called Iodopsin which is responsible for bright light vision and color vision. *(Mnemonic: remember (c)ons =(c)olor vision ) 5. In response to light waves visual pigments(rhodopsin, iodopsin ) belaches to opsin and all trans retinal through a series of photochemical isomerizations. This photolysis of visual pigments initiates visual signals (nerve impulse) in photoreceptor cells that finally helps in vision. 6. Most of the all trans retinal produced by bleaching of visual pigment is isomerized back to 11 cis retinal directly or indirectly via retinol to resynthesize the visual pigment. Part of the all trans retinal is lost and not available for resynthesis of the visual pigments, so it needs to be make up from dietary vitamin A in the form of all trans retinol which is isomerized to 11-cis retinol and then dehydrogenated to 11- cis retinal to form visual pigments. 7. Therefore, continual supply of retinol is essential for normal visual function. Retinol deficiency depresses the resynthesis of visual pigment and interferes with the function of photoreceptors particularly rods causing ****night blindness.**** Now lets discuss about another important topic(light adaptation / dark adaptation) *****Dark adaptation/ light adaptation*****: 1. If a person spends a considerable length of time in bright light and then moves to a dim light environment, gradually visual pigments(rhodopsin) concentration in rods and cons increases. As a result visual threshold decreses and the retina slowly become more sensitive to light so that it can respond to the light of low intensity to allow vision. This is called dark adaptation. 2. In bright light,visual pigments(rhodopsin) in rods and cons undergo photolysis(bleaching) to retinal and opsin reducing its concentration. This causes reduction of the sensitivity of eye to light that raises visual threshold so that eye can respond to the light of high intensity. This is known as light adaptation. 3. When individual goes to dim light the retinal and opsin recombine to build up rhodopsin store necessary for the optimal function of rods and cones at low intensity light. The time needed to build up the necessary rhodopsin store is the time required for dark adaptation.

Animal sources: vitamin A is widely distributed in animal sources rather than plant sources. a. Egg b. Meat c. Fish d. Liver e. Fish oil (e.g. cod liver oil) 2. Plant sources: Vitamin A is also present in some plant sources as well. a. Papaya b. Tomato c. Green leafy vegetables d. Functions of vitamin A Hello learners, I would like to share with you, today, a summary of functions of vitamin A. I hope it will help the students who are currently have a course module about food and nutrition of college and varsity students. I am a medical student and my sir made me to write this notes. Here are they Helps in vision - Helps in synthesizing visual pigments like rods and cons. -rods are involved in dim light vision. -cons are involved in bright light and color vision *dark adaptation time is increased in vitamin A deficiency (MNEMONIC: Cons=Color vision remember c for color vision RoD=Dim light Remember d for dim light vision) Regulation of protein synthesis (Retinol and retinoic acid ) and thus are involved in cell growth and differenciation. Essential to maintain healthy epithelial tissue . Necessary for the synthesis of certain glycoproteins and mucopolysaccharides. Necessary for normal reproduction. Essential for the maintenance of proper immune system. Required for synthesis of cholesterol. Beta carotene functions as antioxidant and reduce the risk of cancer initiated by free radicals and strong oxidants. (Note:oxidants :oxide,peroxide,H2O2 et.)

Life becomes dull.life becomes hell. Lets come to greatest source of vedios:youtube You wont be able to search what your minds say. Right ? Like now, you wanna cook, lets begin searching"how to cook without gases ? Ha ha ha. Dont worrie you will still have some ideas in youtube about that topic.you wanna hack google,you can carry on searching "how to hack google ? " (ha ha ha ) okay, i was kidding. But can you think your lice without youtube. Dont you search daily more than 10 times in youtube. Without internet, how would your youtube run ? Life becomes dull, life becomes hell. Coursera (part 1) In the era of 2020 it is very easy to learn something new, to gather knowledge from worlwide resources simply sitting in your own home. Learning out of the box is very very important for now. Even many people are curious also. For instance in the past if anyone wants to be familiar with programming he must have a fight. But now one can master codes of programming, programming languages java,c++, python through online courses only. There are hundreds of websites which offer online courses with a variety of subjects, topics, life skills and so on. Coursera is one of the best of them. Edx, udemy are also some example. Coursera is best one because here instructors are only from leading universities worlwide (e.g. harvard university, duke university, cambridge university etc.) and from best companies like google, IBM etc. Another fact is that it provides courses free of cost. You can get access to all study materials but if you want certificate you have to lose your pocket. But there is also a solution..... (To be discussed in part 2)
