Anal Fistula

📣📣📣Anal Fistula📣📣📣 🍀যেকোনো বয়সেই হতে পারে কিন্তু infants & middle aged adults দের বেশি হয়। 👨‍⚕️sign symptoms: 1. Defecation এ pain হয়। & এরপরেও 2/1 ঘণ্টা ব্যথা থাকে।

2. মাঝেমাঝে bright red blood দেখা যায়। 👩‍⚕️Causes: Commonly, 1. Constipation 2. Passing large or hard stools 3. Chronic Diarrhoea 4. Childbirth 5. Anal intercourse Also occurs in, 6. Crohn's disease or IBD 7. Anal Cancer 8. HIV 9. Tuberculosis 10. Syphilis 👩‍⚕️Diagnosis: 👉 এতে History নিব আর সাথে physical examination করব। কিন্তু Anal Fissure এ DRE করা contraindicated. Chronic Anal Fissure এ বাইরে থেকে skin tag দেখা যাবে। 👩‍⚕️Treatment: 👉 Conservative Rx💊💉 🩺এখানে stool soft রাখলে কয়েক সপ্তাহের মধ্যে spontaneously heal হয়ে যায়।এজন্য laxatives & fiber rich diet খাবার পরামর্শ করা হয়ে থাকে। এবং প্রতিবার defecation এর পর Hip bath with povisep /viodin solution দিনে অন্তত তিন বার ৩/৪ চামচ povisep sol হালকা কুসুম গরম পানিতে মিশিয়ে ১০-১৫ মিনিটের জন্য বসে থাকতে বলা হয়। এতে sphincter relax হয়ে healing টা quickly হয়।

এছাড়াও 1. Nitroglycerin oinment 2. Topical anaesthetic creams 3. Botulinum toxin injection Surgical Rx✂️ এতকিছুর পরেও healing না হলে সার্জারি তে যেতে হবে। Mainly এতে Lateral Internal Sphincterotomy (LIS) করা হয়। 👩‍⚕️Complications: 👉Untreated থাকলে heal হবে না। So এতে chronic হয়ে যাবে। তখন Recurrence হতে পারে,tearing extend করে surrounding muscle এ spread করতে পারে।
