These are white, crystalline, odorless powder with a faintly bitter taste which act as a central nervous system depressant...🤕🤕 ✅Classification : Duration এর উপর ভিত্তি করে একে ৪ ভাগে ভাগ করা যায়- 🔺Ultra short acting. 🔺Short acting. 🔺Intermediate acting 🔺Long acting. এবার একটু হালকা করে বিস্তারিত জেনে নেই...😇😇😇 🉑Ultra short acting :✊ 🔰Onset of action ➡ Immediate. 🔰Duratuion ➡ 5 - 10 min. 🔰Fatal dose ➡1.5 - 2 gm 🔰Example ➡ Thiopental sodium. 🉑Short acting :✊ 🔰Onset of action ➡ 10 - 20 min. 🔰Duratuion ➡ 1 - 3 hrs. 🔰Fatal dose ➡ 1.5 - 2 gm. 🔰Example ➡ Amobarbitone, Pentobarbitone, Cyclobarbitone. 🉑Intermediate acting :✊✊ 🔰Onset of action ➡ 30 min - 1 hr. 🔰Duratuion ➡ 3 - 6 hrs. 🔰Fatal dose ➡ 2 - 3 gm. 🔰Example ➡ Butobarbitone. 🉑Long acting : ✊✊💪 🔰Onset of action ➡ 2 hr. 🔰Duratuion ➡ 6 - 12 hrs. 🔰Fatal dose ➡ 4 - 7 gm. 🔰Example ➡ Phenobarbitone. ✅barbiturate এর কি কি effects আছে ? 😊😊😊 🔺CNS effects :✔ sedation ✔ Hypnosis ✔Anaesthetic effect. ✔ Anti convulsive effect. ✔ ↓ skeletal NMJ transmission. 🔺CVS effects :✔ Hypotension ✔Tachycardia ✔Heart failure. 🔺Respiratory effects : ✔↓sensitivity to CO2 ✔Medullary depression ✔Respiratory failure. 🔺GIT effects :✔↓nocturnal HCL secretion. ✔↓other GIT disturbance. তবে এটি সবচেয়ে বেশি affected করে বেচারা central nervous system কে...😢😢😢 ✅Acute barbiturate poisoning হলে করণীয় কি? 🤔 ✳patient কে immediately hospitalised করতে হবে। ✳ABCD resuscitation & O2 inhalation. ✳If conscious 👉 Induced vomiting by either trickle of the pharynx or by giving saltwater to drink. ✳If conscious 👉 Gastric lavage should be given if the patient is hospitalized within 4 hours✔✔ ✳Enema & Laxative. ✳Hypotension or shock 👉Inj.Adrenaline/Dopamine. ✳Intravenous fluid 👉 5%DNS(1L) & 5%DA(1L) @30 drops/min in 24 hours. ✳Forced diuresis. ✳Acidosis ✳Dialysis(if necessary). ✳Antibiotic. ✳If no improvement 👉 Inj .Bemegride 10 mg (I/V) ✳General care 👉 Maintenance of intake-output chart, monitoring of vital signs. 🕵🕵টুকিটাকিঃ👽👽 ✅Barbiturate blister কি? It is a rare condition (6%) of severe acute barbiturate poisoning, where irregular shaped erythematous patches (blister) containing clear serous fluid, may appears in the skin. 🔺Sites : Interdigital cleft, inner aspects of the knee, buttocks, calves & forearms কারণ এ সব জায়গায় ২টি surface এর মধ্যে pressure থাকে। 😐😐 কিন্তু এটি খুব দ্রুতই rupture হয়ে শুকিয়ে যায়। ☺ ✅Barbiturate automatism কি? 🤥🤥 নামেই কিছুটা অনুমেয়। patient এর মধ্যে drug নেওয়ার একটি automation তৈরি হয়।ফলে সে বার বার drug নিতেই থাকে। 🔺mechanism ➡ intrinsic anterograde amnesiac effect. 🔺Importance ➡ more pain & sleeplessness causes more disorientation leading to pseudo - suicide. 🔺Barbiturate ছাড়া অন্য কোনো drug এ কি automatism হতে পারে? ✔পারে। 👉Midazolam & other benzodiazepines. ✅যদি patient মারা যায় তাহলে কি কি Post mortem findings পাওয়া যাবে? 😥😥😥 🔺External : 🔹Cyanosis 🔹Congested face 🔹Prominent postmortem staining 🔺Internal : 🔹Stomach → congested, parts of tablets may present 🔹Lungs →congested, edematous with hemorrhagic spots under the surface. 🔹Kidneys →may show tubular degeneration. 🔹Brain & other organs are congested.
