Man died inside soakaway. Who's at fault?

How has been your weekend and the holiday so far? Hope you're having the best of time and commemoration of our Lord Jesus Christ?
I came back from work at my usual hour, very tired and hungry, I turned on gas and did a quick noodles. I ate and took my chair outside to receive fresh air while I think of a topic to write. I was scrolling on nairaland when I met a disturbing news about a man that slipped inside a soakaway and instantly passed on from there.

According to the report, they said the man and on others we on a queue to receive a medical check, when this man got tired, he decided to leave the queue to search for a cool place to sit and rest for a while, and why going he was moving he stepped on a soakaway and slipped down. The odd occurrence got the attention of the people and they put measures to bring him out, they brought him to see that he's dead already. OMG!
A tragic incident occurred at Oredo Primary Health Centre in Oredo Local Government Area of Edo State, as an unidentified man died after falling inside a soakaway, IgbereTV reports.
The incident occurred shortly after the middle-aged man left the long queue for the registration for free medical exercise to urinate.
Eyewitnesses said he mistakenly stepped on a soakaway within the premises and slipped inside. It wad reported that there was no caution sign around the dilapidated soak away which has been covered with grasses.
The incident caused panic among the health workers and in-patients who were helpless. It was further gathered that the victim was brought out dead from the soakaway with the aid of an excavator four hours later.

A lot of people commented and dished out their anger in them, blaming the management of the health care in particular.
Who's at fault?
To me, the blame shouldn't not only be pushed down to the management of the facility but also individuals and the government should be blasted as well. Let me put it down in a more convincing note.
The health center and management is at fault, since they know that the soakaway is not functional anymore and it's dilapidating, they supposed to have placed a warning signal on it because on a normal circumstances, 70% of Nigerians walk on top of soakaway without fear of slipping inside. Yeah, I'm very fond of that. No warning/danger sign was placed there and they were fully aware because I know they used it till they abandoned it.
Maybe the management has been procrastinating on fixing it, but why must they procastinate on such crucial case. It's a health care center for God's sake and should prioritize life before any other thing. You see now, instead of them Saving life, they are not the ones taking life.
Individuals are to be blamed too; As humans(yes, me and you), we don't take alot of things serious because maybe a problem is not affecting us directly. We are too careless on things that matters to life. Just like my region here, a lot of us still fancy this common saying "disease can not kill an African man"(means we are too strong to be taken down by some challenges).
At this point, I'm not trying to attack the man that fell inside the soakaway but to all individuals. Let me bring it clearer to us, do you know that anybody in the health Care centre could have placed a warning signal on the dilapidated soakaway? But no, nobody cared to take responsibility because it's not their job (waiting for the boss to do it).
This thing happens in our individual environment and most times a good number of us feels "I don't care". It doesn't cost a dime to use a red rope to fence a danger zone to prevent people from passing through there. It doesn't cost a thing to uses a stick to put on a trench to give people warning signal.
I remember those times we used to go to farm, when the farmland is freshly burnt my dad would move around the farmland to see if there's any hole from a dead/decayed tree, once he sees one he uses a long stick to place there acting as a sign of danger so we won't fall in.
If all of us could be cautious of our environment to act instantly to some dangers, life will be sweet and unnecessary death will be prevented. We should take it down to heart to maintain some of these social facilities on our own without waiting for the government. We depend too much on he government!
Now to the government; The government has a little or no touch on this, the major blame should be on the management. But my point is, the government is fond of procastinating attention when they're needed. There's possibility that the government had been informed to repair the soakaway but they choose to procastinate with their pattern of passing through many paper work. You get my point?
I don't know about your country but such cases in my country, the government give slow attention untill the situation becomes 100% worst. They would tell you to write letter to office A, B to C and after that, a delegate would be sent to inspect it to see if what's written is actually true or not,,,to avoid "wasting" fund. Lol.
When I'm mean Government, I don't meet the presidential sector of the governor of the State, I mean those offices that oversee these functions. Ofcourse they have governmental offices they're connected to unless the facility/company is on a one man's business. But even at that, the government still have a connection because they authorised such facility.
My take on this
The management of the health center should be arrested and charged to court. Let them learn a bit of lesson and send a warning message across to the country. Let people learn to attend to simple and basic needs of our environment.
It's just like how our dear planet Earth is suffering and crying for help, we(humans) are majorly the cause of what the earth is passing through today. We know the basic solutions(such as planting of plants and trees) we can do but we are not ready, we are waiting for the leaders to do the planting for us.
Let's adjust please.
